新型全视网膜激光光凝治疗对增生型糖尿病视网膜病变患者视网膜前膜相关因子的影响

Effect of new whole-retina laser photocoagulation on factors related to the anterior retinal membrane in patients with proliferative diabetic retinopathy

目的探讨新型全视网膜激光光凝(PRP)术治疗增生型糖尿病视网膜病变(PDR)的临床效果。方法选取2020年6月至2022年1月符合条件的76例PDR患者为研究对象,将其按随机数字表分为对照组和观察组,行单盲法对比研究。对比观察2组治疗效果和视力、眼压、黄斑中心凹视网膜厚度(CMT)水平,同时检测并观察患者血清血管内皮生长因子(VEGF)和内皮素-1(ET-1)水平,并统计手术并发症发生情况。结果治疗后,观察组治疗总有效率(93.62%)高于对照组(79.59%)(χ^(2)=4.031,P<0.05)。观察组视力水平高于对照组,眼压、CMT水平低于对照组(P<0.05);术后视网膜前膜因子水平比较,观察组VEGF、ET-1水平低于对照组(P<0.05)。观察组术后并发症发生率(10.64%)低于对照组(26.53%)(χ^(2)=3.977,P<0.05)。结论新型PRP手术在PDR治疗中,对促进患者视力恢复,改善眼压、黄斑水肿,抑制血管新生有明显优势,能够进一步提升临床治疗效果,且具有较高的安全性,临床应用价值较高。

Objective To investigate the clinical effect of new whole-retina laser photocoagulation(PRP)on the treatment of proliferative diabetic retinopathy(PDR).Methods From June 2020 to January 2022,76 eligible PDR patients were selected based on the inclusion criteria.They were randomly divided into control group and observation group according to the random number table,and then the single-blind comparative study was performed.The therapeutic effects,visual acuity,intraocular pressure and macular fovea retinal thickness(CMT)were compared between the two groups.Meanwhile,the serum levels of vascular endothelial growth factor(VEGF)and endothelin-1(ET-1)were detected and observed,and the incidence of surgical complications was statistically analyzed.Results After treatment,the total effective rate of the observation group was significantly higher than that of the control group(93.62%vs 79.59%,χ^(2)=4.031,P<0.05).The visual acuity level of the observation group was significantly higher than that of the control group,and the intraocular pressure and CMT were significantly lower(P<0.05).Postoperative levels of VEGF and ET-1 in the observation group were significantly lower than those of the control group(P<0.05).The incidence of postoperative complications in the observation group was significantly lower than that of the control group(10.64%vs 26.53%,χ^(2)=3.977,P<0.05).Conclusion The new PRP surgery has obvious advantages in promoting the recovery of vision,intraocular pressure and macular edema and inhibiting angiogenesis in the treatment of PDR,which can further improve the clinical therapeutic effect with high safety and clinical application value.
