摘要
目的:探究不同时期予以早产儿维生素D补充对其骨密度的影响。方法:选取2022年1月—2023年1月肇庆市第一人民医院儿科入住新生儿监护病房的60例早产儿为研究对象,按随机数字表法分为常规保健组与早期维生素D干预组,各30例。常规保健组早产儿从出生后的第14天开始连续口服维生素D,早期维生素D干预组早产儿出生后第3天连续口服维生素D。比较两组出生后各纠正月龄胫骨骨密度Z值、身长,纠正月龄6月龄时碱性磷酸酶、25-羟维生素D水平。结果:早期维生素D干预组纠正月龄1、2、3、6月龄胫骨骨密度Z值、身长均高于常规保健组,差异有统计学意义(P<0.05)。出生时,两组碱性磷酸酶、25-羟维生素D水平比较,差异无统计学意义(P>0.05);纠正月龄6月龄时,两组碱性磷酸酶、25-羟维生素D水平均升高,早期维生素D干预组高于常规保健组,差异有统计学意义(P<0.05)。结论:早期阶段予以早产儿维生素D补充能够增强其骨密度,促进机体发育。
Objective:To investigate the effects of vitamin D supplementation given to preterm infants at different periods on their bone mineral density.Methods:Sixty cases of preterm infants admitted to the neonatal care unit of the Department of Pediatrics of Zhaoqing First People's Hospital from January 2022 to January 2023 were selected for the study,and were divided into the conventional health care group and the early vitamin D intervention group according to the method of randomized numerical table,with thirty cases in each group.Preterm infants in the conventional health care group were given oral vitamin D continuously from the 14th day after birth,and preterm infants in the early vitamin D intervention group were given oral vitamin D continuously on the 3rd day after birth.The Z value of tibia bone density and length at each corrected month of age after birth,and the values of alkaline phosphatase and 25-hydroxyvitamin D levels at 6 months of corrected month of age were compared between the two groups.Results:The Z value of tibia bone density and length at corrected months of age 1,2,3,and 6 months of age in the early vitamin D intervention group were higher than those in the conventional health care group,and the difference was statistically significant(P<0.05).At birth,alkaline phosphatase and 25-hydroxyvitamin D levels were compared between the two groups,and the difference was not statistically significant(P>0.05);at the corrected age of 6 months of age,alkaline phosphatase level,25-hydroxyvitamin D level of the two groups increased,the early vitamin D intervention group was higher than the conventional health care group,and the difference was statistically significant(P<0.05).Conclusion:Vitamin D supplementation in preterm infants at an early-stage can enhance their bone density and promote their physical development.
