阴道超声和腹部超声鉴别诊断子宫肌瘤和子宫腺肌病的价值比较

Comparison on Value of Vaginal Ultrasound and Abdominal Ultrasound in Differential Diagnosis of Uterine Fibroids and Adenomyopathy

目的:比较阴道超声和腹部超声用于子宫肌瘤和子宫腺肌病鉴别诊断的价值。方法:选取2018年10月—2023年2月临沂市精神卫生中心收治的疑似子宫肌瘤的患者98例作为研究对象,均进行阴道超声和腹部超声检查,以病理诊断结果为“金标准”,比较两种检查方式对子宫肌瘤和子宫腺肌病的诊断效能。结果:98例疑似子宫肌瘤的患者经病理诊断为子宫肌瘤79例,子宫腺肌病19例。阴道超声检查诊断子宫肌瘤的灵敏度、准确性、阴性预测值高于腹部超声检查,差异有统计学意义(P<0.05);阴道超声诊断子宫腺肌病的灵敏度、准确性和阳性预测值低于腹部超声,差异有统计学意义(P<0.05)。结论:阴道超声在子宫肌瘤中的诊断效能较高,腹部超声在子宫肌腺中的诊断效能较高,必要时需联合两种检查方法鉴别子宫肌瘤和子宫腺肌病。

Objective:To compare the value of vaginal ultrasound and abdominal ultrasound in the differential diagnosis of uterine fibroids and adenomyosis.Methods:A total of ninety-eight patients with suspected uterine fibroids admitted to Linyi Mental Health Center from October 2018 to February 2023 were selected as the study objects,and all of them underwent vaginal ultrasound and abdominal ultrasound.Taking the pathological diagnosis results as the"gold standard",the diagnostic efficacy of the two methods for uterine fibroids and adenomyosis was compared.Results:Of 98 suspected patients with uterine fibroids,79 were pathologically diagnosed with uterine fibroids and 19 with adenomyosis.The sensitivity,accuracy and negative predictive value of vaginal ultrasound in the diagnosis of uterine fibroids were higher than those of abdominal ultrasound,the difference was statistically significant(P<0.05);the sensitivity,accuracy and positive predictive value of vaginal ultrasound in the diagnosis of adenomyosis were lower than those of abdominal ultrasound,and the difference was statistically significant(P<0.05).Conclusion:The diagnostic efficacy of vaginal ultrasound in uterine fibroids is relatively high,and that of abdominal ultrasound in uterine muscle glands is relatively high.If necessary,the two methods should be combined to distinguish uterine fibroids from adenomyosis.
