绳梯式穿刺法和扣眼穿刺法在血液透析患者自体动静脉内瘘穿刺中的应用效果

Effect of Rope Ladder Puncture and Buttonhole Puncture on Autologous Arteriovenous Fistula in Hemodialysis Pa-tients

目的:探讨绳梯式穿刺法和扣眼穿刺法在血液透析患者自体动静脉内瘘中的应用效果。方法:选取2020年5月—2022年1月于佛山市南海区第七人民医院血液室采用自体动静脉内瘘透析的患者70例为研究对象,以随机数字表法分成研究组和对照组,各35例。两组患者均进行血液透析治疗,研究组实施绳梯式穿刺法,对照组实施扣眼穿刺法。比较两组一次性穿刺成功率、穿刺满意度及内瘘并发症发生率。结果:研究组1、3、6、9、12、18个月的一次性穿刺成功率高于对照组,差异有统计学意义(P<0.05);研究组6、12、18个月的穿刺满意度高于对照组,差异有统计学意义(P<0.05);研究组内瘘并发症发生率低于对照组,差异有统计学意义(P=0.031)。结论:绳梯式穿刺法和扣眼穿刺法在血液透析患者自体动静脉内瘘中的应用效果存在差异,绳梯式穿刺可提高一次穿刺成功率及患者的穿刺满意度,降低并发症发生率。

Objective:To investigate the effect of rope ladder puncture and buttonhole puncture on autologous arteriovenous fistu-la in hemodialysis patients.Methods:Seventy patients who underwent autologous arteriovenous fistula dialysis in the Blood room of Foshan Nanhai District Seventh People's Hospital from May 2020 to January 2022 were selected as the study objects,and were divided into the study group and the control group with thirty-five patients in each group by random number table method.Both groups underwent hemodialysis treatment.Rope ladder puncture was performed in the study group,and buttonhole puncture was performed in the control group.The success rate of one-time puncture,puncture satisfaction and the incidence of internal fistula complications were compared between the two groups.Results:The success rate of one-time puncture at 1,3,6,9,12 and 18 months in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05);The puncture satisfaction of the study group at 6,12 and 18 months was higher than that of the control group,the difference was statisti-cally significant(P<0.05);The incidence of fistula complications in the study group was lower than that in the control group,and the difference was statistically significant(P=0.031).Conclusion:There are differences between rope ladder puncture and button-hole puncture in the application of autologous arteriovenous fistula in hemodialysis patients.Rope ladder puncture can improve the success rate of one puncture and the satisfaction of patients,and reduce the incidence of complications.
