超声颈丛阻滞结合气管插管对内膜剥脱术患者脑保护作用

摘要 目的:探讨超声引导下颈丛阻滞结合气管插管全麻对颈动脉内膜剥脱术患者的脑保护作用。方法:选择邢台市第三医院收治的128例缺血性脑卒中患者,比较患者脑组织代谢、氧化应激等指标。结果:观察组患者术后1d和术后3d血清s100-b蛋白和NSE水平明显低于对照组(P<0.05)。观察组患者术后1d、术后3d和术后7d血清SOD和TAC明显高于对照组,观察组患者术后1d、术后3d和术后7d血清MDA明显低于对照组(P<0.05)。结论超声引导下颈丛阻滞结合气管插管全身麻醉应用于颈动脉内膜剥脱术,可有效改善围术期指标,纠正氧化应激反应,减轻脑组织代谢,安全有效。 Objective:To study theeffect of ultrasound-guided superficial cervical plexus block combined with general anesthesia on oxidative stress and brain metabolism in patients undergoing carotid endarterectomy.Methods:A total of 128 patients with ischemic stroke admitted to Xingtai Third Hospitalwere selected,and the brain tissue metabolism,oxidative stress and other indicators of the patients were compared.Results:Serum s100-bprotein and NSE levels in the observation group were significantly lower than those in control group 1d and 3d after surgery(P<0.05).Serum SOD and TAC in observation group were significantly higher than those in control group 1d,3d and 7d after surgery,while serum MDA in observation group was significantly lower than control group 1d,3d and 7d after surgery(P<0.05).Conclusion:Ultrasound-guided cervical block combined with tracheal intubation general anesthesia in carotid endarterectomy can effectively improve perioperative indicators,correct oxidative stress response,and reduce brain tissue metabolism.
