TLRs和sTREM-1与支原体肺炎患儿预后相关性

Relationship between chest CT and serum TLRs,sTREM-1 expression and prognosis in children with mycoplasma pneumonia

目的:研究肺炎支原体肺炎患儿胸部CT与TLRs、sTREM-1表达、预后的关系。方法:选择2019年5月至2022年1月收治的204例肺炎支原体肺炎患儿,根据病情,将患儿分成普通组和重症组,比较两组患儿胸部CT评分、TLRs和sTREM-1水平,Pearson法分析胸部CT评分和TLRs和sTREM-1的相关性。随访6个月,分成预后良好组和预后不良组,Logistic回归分析确定预后不良影响因素,ROC曲线分析诊断效能。结果:重症组患儿CT评分、TLR5及sTREM-1明显高于普通组。Logistic回归分析表明,CT评分、TLR2、TLR4、TLR5和sTREM-1是预后不良的影响因素。ROC曲线表明,联合诊断敏感度和特异性高于单独检测(P<0.05)。结论:胸部CT评分、TLR2、TLR4、TLR5、sTREM-1和Logistic回归分析可应用于支原体肺炎预后不良的预测和诊断。

Objective:To study the relationship between chest CT and expression of TLRs,sTREM-1 and prognosis in children with Mycoplasma pneumoniae pneumonia.Methods:In this study,a total of 204 children diagnosed with mycoplasma pneumoniae pneumonia and admitted between May 2019 and January 2022 were enrolled.Based on the severity of their condition,the patients were categorized into two groups:the normal group and the severe group.Subsequently,a comparative analysis was conducted to assess the levels of chest CT scores,TLRs,and sTREM-1 between the two groups.The Pearson correlation method was employed to evaluate the relationship between chest CT scores and TLRs,as well as sTREM-1.Follow-up assessments were performed over a period of 6 months,classifying the patients into either a good prognosis group or a poor prognosis group.Logistic regression analysis was employed to identify the influencing factors associated with poor prognosis,and the diagnostic efficiency was assessed by constructing ROC curve.Results:CT scores,TLR5 and sTREM-1 in the severe group were significantly higher than those in the common group.Logistic regression analysis showed that CT score,TLR2,TLR4,TLR5 and sTREM-1 were the influencing factors of poor prognosis.ROC curve showed that the sensitivity and specificity of combined diagnosis were higher than those of single detection(P<0.05).Conclusion:Chest CT score,TLR2,TLR4,TLR5,sTREM-1 and Logistic regression analysis can be used to predict and diagnose the poor prognosis of mycoplasma pneumonia.
