腹腔镜下经胆囊管胆总管取石及胆总管切开取石治疗胆总管结石的可行性比较

Comparison of the Feasibility of Laparoscopic Choledocholithotomy and Choledocholithotomy for Choledocholithiasis

目的对比腹腔镜下经胆囊管胆总管取石术与胆总管切开取石术应用于胆总管结石患者的临床效果。方法选取2022年1月—2023年6月本院收治的80例胆总管结石患者为研究对象,并按照随机数字表法分成对照组和观察组,每组40例,对照组采取胆总管切开取石术,观察组采用腹腔镜下经胆囊管胆总管取石术。对比两组在围术期指标、炎症指标、肝功能指标以及并发症情况方面的差异。结果与对照组相比,观察组的围术期相关指标均较佳,炎症相关指标数值均较低,肝功能相关指标均较优,且术后常见并发症的发生率较低(P<0.05)。结论与胆总管切开取石术相比,胆总管结石患者经过腹腔镜下经胆囊管胆总管取石术治疗后的围术期相关指标用时较短,炎症水平较低,肝功能水平有了明显改善,且术后并发症的发生风险较低,安全性佳,疗效确切。

Objective To compare the clinical effects of laparoscopic choledochotomy and choledochotomy in patients with choledocholithiasis.Methods All 80 patients were diagnosed with choledocholithiasis.The first patient came to our hospital in January 2022,and the last patient came to our hospital in June 2023.Two groups were set up in this study,namely the control group and the observation group.There were 40 subjects in each group.Random number table method was the grouping method of this study,choledochotomy was the treatment plan of the former,and laparoscopic choledochotomy was the treatment plan of the latter.The differences in perioperative indexes,inflammatory indexes,liver function indexes and complications between the two groups were compared.Results Compared with the control group,the perioperative relevant indexes were better,the inflammation-related indexes were lower,the liver function related indexes were better,and the incidence of common postoperative complications was lower in the observation group(P<0.05).Conclusion Compared with choledochotomy and lithotomy,Patients with choledocholithiasis who underwent laparoscopic choledocholithotomy had shorter perioperative relevant indexes,lower inflammation level,significantly improved liver function level,and lower risk of postoperative complications,good safety and accurate efficacy.
