凝血因子Ⅻ缺乏致患者活化部分凝血活酶时间延长的临床分析

Clinical analysis of activeated partial thromboplasting time prolongation in patients with coagulation factorⅫdeficiency

目的探讨凝血因子Ⅻ缺乏导致患者活化部分凝血活酶时间(APTT)延长的临床分析。方法收集2019年6月至2021年12月于滨州医学院附属医院查体或就诊患者的血液凝血功能检查结果,筛查出3例APTT单独延长的血凝结果,另选取同期20名血液凝血功能检查结果正常者作为对照组,进行APTT纠正并分析试验结果及凝血因子活性检测结果。结果3例患者APTT的检测结果分别为159.8、142.3、158.1 s。对照组血浆混匀后测得APTT结果分别为26.0、26.6、23.5 s。患者血浆与正常混匀血浆1∶1混匀后测得APTT结果分别为28.7、28.1、26.4 s;混匀血浆37℃孵育1 h后,测得APTT结果分别为28.5、28.0、26.6 s;3例患者凝血因子Ⅻ(FⅫ)活性分别为8.0%、5.4%、1.2%。结论患者单独APTT延长,但无其他与血液系统相关的临床症状,也无凝血相关的疾病,不影响正常的生理功能,查体或无意中发现,临床治疗不建议进行补充凝血因子等特殊治疗,但如果存在原发病,仍需密切关注。

Objective To investigate the clinical analysis of activeated partial thromboplasting time prolongation in patients with coagulation factorⅫdeficiency.Methods Blood coagulation function test results of patients who underwent physical examination or visited the Affiliated Hospital of Binzhou Medical College from June 2019 to December 2021 were collected,and 3 cases with APTT alone prolonged hemagglutination results were screened,APTT correction was carried out,another 20 subjects with normal blood coagulation function test results in the same period were selected as the control group,and the test results and coagulation factor activity detection results were analyzed.Results The APTT test results of three patients were 159.8,142.3 and 158.1 s,respectively,the APTT results was measured after plasma mixing in the control group were 26.0,26.6 and 23.5 s,respectively.The results of APTT were 28.7,28.1 and 26.4 s after 1:1 mixing of plasma of patients and normal subjects,after incubation at 37℃for 1 h,APTT results were 28.5s,28.0s and 26.6 s,respectively.Coagulation factorⅫ(FⅫ)activity of the three patients were 8.0%,5.4%,and 1.2%,respectively.Conclusion The patient has prolonged APTT alone,but has no other clinical symptoms related to the blood system or clotting related diseases,which does not affect normal physiological functions,if physical examination or accidental discovery,it is not recommended to supplement coagulation factor and other special treatments in clinical treatment,but if there is a primary disease,it still needs to be paid close attention.
