奥扎格雷钠注射液联合亚低温治疗对颅脑损伤并发急性脑梗死患者神经功能及血流动力学的影响

摘要 目的探讨奥扎格雷钠注射液联合亚低温治疗在颅脑损伤并发急性脑梗死患者中的治疗效果。方法选取2019年11月至2021年11月滨州市第二人民医院收治的82例颅脑损伤并发急性脑梗死患者作为研究对象,按照随机数字表法分为对照组与观察组,各41例。对照组采用奥扎格雷钠注射液治疗,观察组在对照组基础上联合亚低温治疗。比较两组临床疗效、神经功能指标、血流动力学指标及不良反应发生情况。结果观察组治疗总有效率为95.12%,高于对照组的80.49%,差异有统计学意义(P<0.05)。治疗前,两组脑元神经性营养因子(BDNF)、髓鞘碱性蛋白(MBP)、神经元特异性烯醇化酶(NSE)及缺血修饰清蛋白(IMA)水平比较差异无统计学意义;治疗后,两组BDNF水平均高于治疗前,MBP、NSE、IMA水平均低于治疗前,且观察组BDNF水平高于对照组,MBP、NSE、IMA水平均低于对照组,差异有统计学意义(P<0.05)。治疗前,两组全血高切黏度(HBV)、血浆黏度(PV)、全血低切黏度(LBV)比较差异无统计学意义;治疗后,两组HBV、PV、LBV均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论亚低温治疗联合奥扎格雷钠注射液在颅脑损伤并发急性脑梗死治疗中的效果良好,可改善患者血流动力学指标及神经功能,安全性高。 Objective To explore the effect of ozagrel sodium injection combined with mild hypothermia therapy on neurological function and hemodynamics in patients with craniocerebral injury complicated with acute cerebral infarction.Methods 82 patients with traumatic brain injury complicated with acute cerebral infarction admitted to Binzhou Second People's Hospital from November 2019 to November 2021 were selected as the study subjects,and they were divided into the control group and the observation group according to the random number table method,with 41 cases in each group.The control group received treatment with ozagrel sodium injection,while the observation group received combined mild hypothermia treatment on the basis of the control group,the clinical efficacy,neurological function indicators,hemodynamic indicators,and adverse reactions were compared between two groups.Results The total effective rate in the observation group was 95.12%,which was higher than 80.49%in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in the levels of brain-derived neurotrophic factor(BDNF),myelin basic protein(MBP),neuron specific enolase(NSE),and ischemic modified albumin(IMA)between the two groups;after treatment,the levels of BDNF of the two groups were higher than before treatment,while the levels of MBP,NSE and IMA were lower than before treatment,and the BDNF level in the observation group was higher than that in the control group,while the levels of MBP,NSE,and IMA were lower than those in the control group,and the differences were statistically significant(P<0.05).Before treatment,there was no statistically significant difference in whole high blood viscosity(HBV),plasma viscosity(PV),and whole low blood viscosity(LBV)between the two groups;after treatment,the HBV,PV,and LBV of the two groups were lower than those before treatment,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups.Conclusion Mild hypothermia treatment combined with ozagrel sodium injection is effective in the treatment of craniocerebral injury complicated with acute cerebral infarction,can improve hemodynamic indicators and neurological function,and has high safety.
