MRI与高频超声在乳腺癌诊断中的应用分析

摘要 目的分析MRI与高频超声(HFUS)在乳腺癌诊断中的应用价值。方法选取2021年7月至2022年7月于萍乡市人民医院就诊80例疑似乳腺癌患者作为研究对象,均行MRI、HFUS、病理检查,比较MRI、HFUS及MRI联合HFUS的诊断效能,分析乳腺癌患者影像学特征。结果80例疑似乳腺癌患者中,病理检查确诊乳腺癌70例;MRI联合HFUS检查的诊断准确度、灵敏度、阴性预测值均高于MRI、HFUS单一检查,差异有统计学意义(P<0.05),MRI与HFUS检查诊断准确度、灵敏度、阴性预测值比较差异无统计学意义;MRI、HFUS及MRI联合HFUS的特异度、阳性预测值比较差异无统计学意义。乳腺癌MRI特征:浅分叶状、不规则肿块;边缘存在短细毛刺,边界模糊不清,平扫时T_(2)WI信号时呈多样化,而T_(1)WI为低信号,采用增强扫描时为环形或不均匀性强化。HFUS特征:一般为低回声,实性肿物,边缘为“蟹足样”“毛刺征”,无规则形态,伴有钙化,且呈簇状、点状散在分布,肿块有丰富血流信号,内部不均匀低回声,存在穿枝状血流。结论MRI、HFUS在诊断乳腺癌时各有优势,且二者联合可提高诊断准确率及灵敏度,值得临床推广应用。 Objective To analyze the application value of MRI and high-frequency ultrasound(HFUS)in the diagnosis of breast cancer.Methods A total of 80 patients with suspected breast cancer who received treatment in Pingxiang People's Hospital from July 2021 to July 2022 were selected as the study subjects,all patients underwent MRI,HFUS and pathological examination,and the diagnostic efficiency of MRI,HFUS and MRI combined with HFUS were compared,and the imaging characteristics of breast cancer patients were analyzed.Results Among 80 suspected breast cancer patients,70 cases of breast cancer were confirmed by pathological examination.The accuracy,sensitivity and negative predictive value of MRI combined with HFUS were higher than those of MRI and HFUS,and the differences were statistically significant(P<0.05),but there was no significant difference in accuracy,sensitivity and negative predictive value between MRI and HFUS.There was no significant difference in the specificity and positive predictive value of MRI,HFUS and MRI combined with HFUS.MRI features of breast cancer:shallow lobed,irregular mass;there are short and thin burrs at the edge,the boundary is blurred,and the T_(2)WI signal is diverse in plain scan,while the T_(1)WI signal is low,and the ring or uneven enhancement is used in enhanced scan.HFUS features:generally hypoechoic,solid mass with"crab-foot"and"burr"edges,irregular shape,accompanied by calcification,and scattered in clusters and dots,the mass has rich blood flow signals,uneven internal low echo,and perforated blood flow.Conclusion MRI and HFUS have their own advantages in the diagnosis of breast cancer,and the combination of MRI and HFUS can improve the diagnostic accuracy and sensitivity,which is worthy of clinical promotion and application.
