摘要
目的探究三维超声定量研究对育龄期PCOS的诊断价值。方法将2021年6月—2022年12月在中山市博爱医院生殖内分泌科诊断为PCOS的40名育龄期妇女招募为研究组,并随机选择40名健康对照者为对照组。研究组和对照组分别进行阴道三维及二维超声检查,对比两组诊断效果。结果研究组子宫内膜厚度高于对照组,卵巢体积、卵泡容积、平均卵泡数目均大于对照组,两组数据比较差异明显,具有统计学意义(P<0.05)。研究组Ⅵ、FI、VFI均高于对照组,两组数据比较差异有统计学意义(P<0.05)。ROC曲线分析显示经阴道二维超声诊断参数(卵巢体积和人工卵泡数)对PCOS的诊断性能,曲线下面积分别为0.716和0.664。经阴道超声三能多普勒结合彩色能量多普勒技术的诊断参数(卵巢体积、自动卵泡计数、Ⅵ、FI、VFI)对PCOS具有一定的诊断性能,曲线下面积分别为0.830、0.805、0.776、0.708、0.730。结论经阴道三维超声定量测量可以客观地分析PCOS的血供和血流,为PCOS的诊断提供具体的参考值。
Objective To explore the diagnostic value of three-dimensional ultrasound quantitative study on PCOS in childbearing age.Methods A total of 40 women of reproductive age diagnosed with PCOS in Department of Reproductive Endocrinology,Zhongshan Boai Hospital from June 2021 to December 2022 were recruited as the study group,and 40 healthy controls were randomly selected as the control group.The study group and the control group underwent three-dimensional and two-dimensional vaginal ultrasonography respectively.Results The endometrial thickness of the study group was higher than that of the control group,the ovarian volume,follicle volume and average number of follicles were higher than that of the control group,the data of the two groups were significantly different(P<0.05),theⅥ,FI and VFI of the study group were higher than that of the control group,the data of the two groups were statistically significant(P<0.05).ROC curve analysis showed that the diagnostic performance of transvaginal two-dimensional ultrasound diagnostic parameters(ovarian volume and artificial follicle number)on PCOS,and the area under the curve were 0.716 and 0.664.The diagnostic parameters(ovarian volume,automatic follicle count,Ⅵ,FI,VFI)of transvaginal ultrasound combined with color energy doppler technique had certain diagnostic performance for PCOS,and the areas under the curve were respectively 0.830,0.805,0.776,0.708,0.730.Conclusion Transvaginal threedimensional ultrasound quantitative measurement can objectively analyze the blood supply and blood flow of PCOS,and provide specific reference value for the diagno sis of PCOS.
