腹腔镜逆行胆囊切除术治疗复杂胆囊结石的效果分析

Effect Analysis of Laparoscopic Retrograde Cholecystectomy in the Treatment of Complex Gallstones

目的探讨腹腔镜逆行胆囊切除术治疗复杂胆囊结石患者后的效果。方法本次试验截取镇江市丹徒区人民医院在2020年1月—2023年3月收治的60例复杂胆囊结石患者作为对象,按照数字随机方法分为对照和观察,每组30例。对照组实施腹腔镜顺行胆囊切除术治疗,观察组实施腹腔镜逆行胆囊切除术治疗。对比两组临床相关指标,炎症因子水平[C反应蛋白(CRP)、白细胞介素-6(IL-6)、降钙素原(PCT)],并发症发生率。结果观察组手术时间、肛门排气时间、下床活动时间、住院时间较对照组更短,术中出血量较对照组更少,组间差异有统计学意义(P<0.05)。两组对比炎症因子水平,观察组CRP、IL-6、PCT较对照组更低,组间差异有统计学意义(P<0.05)。两组对比并发症发生率,观察组低于对照组,组间差异有统计学意义(P<0.05)。结论通过腹腔镜逆行胆囊切除术治疗复杂胆囊结石患者,可以缩短手术时间、肛门排气时间、下床活动时间、住院时间,减少术中出血量,缓解炎症,降低并发症发生率,值得推广。

Objective To investigate the effect of laparoscopic retrograde cholecystectomy in the treatment of complex gallstones.Methods In this experiment,60 patients with complex cholecystolithiasis admitted to Dantu District People's Hospital of Zhenjiang City from January 2020 to March 2023 were selected as data analysis objects,and were grouped by numerical random method,which was divided into control group and observation group,30 cases were the number of each group.The control group was treated with laparoscopic anterograde cholecystectomy,and the observation group was treated with laparoscopic retrograde cholecystectomy.The clinical related indexes were compared between the two groups.The levels of inflammatory factors,including C-reactive protein(CRP),interleukin-6(IL-6)and procalcitonin(PCT);were compared between the two groups.The incidence of complications was compared between the two groups.Results In the observation group,the operation time was(60.34±10.12)min,the anal exhaust time was(24.20±5.30)h,the getting out of bed time was(13.05±2.33)h the hospitalization time was(4.25±1.20)d,and the intraoperative blood loss was(40.07±8.67)mL.There was less,and the difference was significant(P<0.05).Compared the levels of inflammatory factors between the two groups,CRP(4.03±2.98)mg/L,IL-6(21.05±5.68)pg/mL and PCT(0.03±0.01)ng/mL were lower in the observation group,and the differences were significant(P<0.05).The incidence of complications between the two groups was 3.33%in the observation group and 20.00%in the control group,which was lower in the observation group than in the control group,the difference was significant(P<0.05).Conclusion Laparoscopic retrograde cholecystectomy in the treatment of patients with complex gallstones can shorten the operation time,anal exhaust time,get out of bed activity time,hospital stay time,reduce intraoperative blood loss,relieve inflammation and reduce the incidence of complications,which is worthy of promo tion.
