哌拉西林钠他唑巴坦钠在老年重症肺炎患者早期经验性治疗中的应用研究

Application Research of Piperacillin Sodium and Tazobactam Sodium in Early Empirical Treatment of Elderly Patients with Severe Pneumonia

目的探究哌拉西林钠他唑巴坦钠在老年重症肺炎患者早期经验性治疗中的应用效果。方法选择本院2022年6月—2023年6月收治的重症肺炎老年患者60例为研究对象,根据治疗方案不同分为对照组(n=28)和观察组(n=32),两组均留取痰样本行痰培养+药敏试验,在结果未出之前对照组采用亚胺培南西司他丁治疗,观察组采用哌拉西林钠他唑巴坦钠,比较治疗3d症状好转率、药物不良反应发生率以及治疗前后肺炎指数(PSI)评分、血清炎症因子变化。结果用药治疗3d后,两组好转率、不良反应发生率无明显的差异(P>0.05)。两组治疗后的肺炎指数PSI评分、血清CRP、PCT水平均较治疗前有所下降,但组间差异无统计学意义(P>0.05)。观察组经验性抗感染治疗成本-效果比低于对照组。结论在老年重症肺炎患者早期经验性治疗中应用哌拉西林钠他唑巴坦钠治疗有助于降低机体炎症水平,改善PSI评分,且治疗成本较低,值得推广应用。

Objective To explore application effect of piperacillin sodium and tazobactam sodium in early empirical treatment of elderly patients with severe pneumonia.Methods The paper chose 60 elderly patients with severe pneumonia admitted to our hospital from June 2022 to June 2023 as research subjects,and divided them into control group(n=28)and observation group(n=32)according to different treatment plans.Both groups were treated with sputum culture and drug sensitivity testing based on sputum samples.Before the results were available,control group was treated with imipenem cilastatin,while observation group with piperacillin sodium tazobactam sodium.Improvement rate of symptoms after 3 days of treatment,incidence of adverse drug reactions and changes in Pneumonia Index(PSI)scores and serum inflammatory factors were compared before and after treatment.Results After 3 days of medication treatment,there was no significant difference in improvement rate and incidence of adverse reactions between two groups(P>0.05).Pneumonia index PSI scores,serum CRP,and PCT levels between two groups were lower than before treatment,there was no statistically significant difference between the groups(P>0.05).Cost-effect ratio of empirical anti infection treatment in observation group was lower than control group.Conclusion Piperacillin sodium and tazobactam sodium can reduce level of inflammation in the body,improve PSI scores in early empirical treatment of elderly patients with severe pneumonia,with lower treatment costs,which is worthy of promotion and application.
