腹股沟疝无张力修补术后补片感染的临床分析

Clinical Analysis of Patch Infection after Tension-free Repair of Inguinal Hernia

目的分析腹股沟疝无张力修补术后补片感染的原因和干预措施。方法筛选2019年1月—2022年12月本院收治的20例腹股沟疝无张力修补术后补片感染患者作为观察组,另选择20例未感染患者作为参照组,对术后补片感染的相关因素进行分析,并分析干预措施。结果单因素分析:观察组BMI>25kg/m^(2)、手术时间>1h、高血压、糖尿病、年龄>70岁发生率均高于对照组,组间差异有统计学意义(P<0.05)。观察组与对照组在性别、饮食干预、运动指导、疾病知识方面发生率对比无统计学意义(P>0.05)。Logistic回归分析显示BMI、手术时间、高血压、糖尿病、年龄属于引发腹股沟疝无张力修补术后补片感染的危险因素。同时对外科手术治疗结果分析,治疗后TNF-ɑ、CRP、IL-6均显著低于治疗前(P<0.05)。结论腹股沟疝无张力修补术后补片感染与年龄、肥胖、2型糖尿病、高血压和手术时间之间存在联系,需要在术前做好准备工作,同时结合患者具体感染情况确定是否须要进行外科手术干预,更好地控制感染。

Objective To analyze the causes and intervention measures of patch infection after tension-free repair of inguinal hernia.Methods In this experiment,20 patients with patch infection after tension-free repair of inguinal hernia were selected as the observation group from January 2019 to December 2022,and another 20 uninfected patients were selected as the reference group.The related factors of postoperative patch infection were analyzed,and the intervention measures were analyzed.Results Univariate analysis:The incidence of BMI>25kg/m^(2),operation time>1h,hypertension,diabetes and age>70 years in observation group were all higher than those in control group(P>0.05).Logistic regression analysis showed that BMI,operation time,hypertension,diabetes and age were the risk factors for patch infection after tension-free repair of inguinal hernia.There was no statistical significance between the observation group and the control group in terms of gender,dietary intervention,exercise guidance and disease knowledge.Meanwhile,analysis of surgical treatment results showed that TNF-a,CRP and IL-6 after treatment were significantly lower than before treatment(P<0.05).Conclusion Patch infection after tension-free inguinal hernia repair is associated with age,obesity,type 2 diabetes,hypertension and operation time.Preoperative preparation is required,and surgical intervention is required according to the specific infection situation of patients to better control infection.
