抗蝮蛇毒血清与抗五步蛇毒血清在竹叶青蛇咬伤治疗中的应用研究

摘要 目的比较不同抗蛇毒血清治疗竹叶青蛇咬伤的应用价值。方法选取2021年3月—2023年1月本院收治的58例竹叶青蛇咬伤患者为研究对象,随机分为对照组和观察组,每组29例。对照组采用抗蝮蛇毒血清,观察组采用抗五步蛇毒血清。比较两组预后结局的差异,并比较两组治疗前后凝血指标的差异。结果观察组并发症发生率低于对照组,伤口出血缓解时间、伤口肿胀消退时间、住院时间短于对照组,差异有统计学意义(P<0.05);治疗前,两组PT、APTT、FIB相比,差异无统计学意义(P>0.05);治疗后,观察组PT、APTT短于对照组,FIB高于对照组,差异有统计学意义(P<0.05)。结论抗五步蛇毒血清在竹叶青蛇咬伤中的治疗效果优于抗蝮蛇毒血清,可降低并发症发生率,改善患者的凝血功能,可推广使用。 Objective To compare application value of different anti snake venom serums in treatment of trimeresurus stejnegeri bites.Methods The paper chose 58 patients with trimeresurus stejnegeri bites from March 2021 to January 2023,and divided them into two groups randomly.Control group was treated with anti viper venom serum,while observation group with agkistrodon acutus antivenin.Difference in prognostic outcomes was compared between two groups,difference in coagulation indicators before and after treatment was compared between two groups.Results Incidence of complications in observation group was lower than control group,time of wound bleeding relief,wound swelling resolution,and hospital stay was shorter than control group,with significant difference(P<0.05).Before treatment,there was no significant difference in PT,APTT,and FIB between two groups(P>0.05).After treatment,observation group had shorter PT and APTT than control group,and FIB was higher than control group,with significant difference(P<0.05).Conclusion Agkistrodon acutus antivenin can achieve better curative effect on trimeresurus stejnegeri bite than antiviper venom serum,can reduce incidence of complications,improve coagulation function of patients,which is worthy of promotion and application.
