腹腔镜结肠癌根治术对老年局部进展期结肠癌患者术后并发症发生率及免疫功能的影响

Effect of Laparoscopic Radical Resection of Colon Cancer on Postoperative Complication Rate and Immune Function in Elderly Patients with Locally Advanced Colon Cancer

目的探究腹腔镜结肠癌根治术对老年局部进展期结肠癌患者术后并发症发生率及免疫功能的影响。方法选取2020年1—12月本院收治的70例老年局部进展期结肠癌患者为研究对象,以随机数字表法分为观察组、对照组两组,每组35例。分别为对照组选择开腹的手术治疗,观察组则实施以腹腔镜辅助下结肠癌根治术。观察以及对比两组免疫功能指标改变情况,此外针对术后各组所发生不良反应情况予以记录,统计类型并计算总体发生情况。结果术前相关免疫指标项目(CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))组间的差异无统计学意义(P>0.05);在术后3个月,免疫指标相比较术前均下降,观察组相较于对照组高,差异具有统计学意义(P<0.05)。观察组术后并发症率(8.57%)相较于对照组(28.57%)更低,组间相比较差异具有统计学意义(P<0.05)。结论腹腔镜结肠癌根治术治疗老年局部进展期结肠癌,对免疫功能的影响小,术后并发症少,值得临床广泛推广及应用。

Objective To explore the effects of laparoscopic radical resection of colon cancer on postoperative complication rate and immune function in elderly patients with locally advanced colon cancer.Methods 70 cases of local advanced colon cancer in elderly patients were selected from our hospital from January to December 2020.The patients were divided into two groups according to the principle of random number table,which were observation group and control group respectively,with 35 patients in each group.All the surgical treatment methods were given to each group,respectively for the control group to choose open surgery treatment,the observation group was implemented with laparoscopic assisted radical resection of colon cancer.The results of the study were mainly to observe and compare the changes in the indexes of immune function.In addition,the adverse reactions occurred in each group after surgery were recorded,statistical types and the overall occurrence were calculated.Results As for the immunoassay items measured before surgery,the main test results showed that there was no significant difference between the two groups in CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)(P>0.05).At 3 months after surgery,the measured immune indicators of CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)were compared with the preoperative moment to decrease,in addition,the observation group was higher than the control group,the results between the groups were compared,the difference was statistically significant(P<0.05).Compared with the control group,the incidence of postoperative complications in the observation group was lower,the incidence of 8.57%was lower than that 28.57%in the control group,and the difference between the groups was statistically significant(P<0.05).Conclusion Laparoscopic radical resection of colon cancer in the treatment of local advanced colon cancer in the elderly has little effect on immune function and fewer postoperative complications,which is worthy of widespread clinical promotion and application.
