局麻下肋骨骨折内固定术的临床应用研究

Clinical Application Research of Internal Fixation for Rib Fractures Under Local Anesthesia

目的探讨对行肋骨骨折内固定术患者应用局部麻醉的效果。方法选择2021年1月—2023年6月本院收治的60例行肋骨骨折内固定术患者为研究对象,并随机分成试验组与对照组,每组30例。对照组应用气管插管麻醉方式,试验组应用局部麻醉方式。比较两组生化指标、免疫指标、恢复指标及术后并发症情况。结果术后第1d各组WBC、NEUT、CRP均较术前1d提高,试验组低于对照组(P<0.05);术后第4d、第7d的各指标较术后第1d降低,试验组均低于对照组(P<0.05)。术后第1d、4d、7d各组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均较术前1d提高,试验组高于对照组(P<0.05);术后第1d、4d、7d各组CD8^(+)均较术前1d降低,试验组低于对照组(P<0.05)。试验组患者恢复指标优于对照组(P<0.05)。结论肋骨骨折内固定术治疗者,应用局部麻醉可降低对机体生活指标及免疫指标影响,促进术后恢复。

Objective To explore effect of local anesthesia on patients with rib fracture internal fixation surgery.Methods The paper chose 60 patients with internal fixation surgery for rib fractures and divided them into experimental group and control group randomly,with 30 cases in each group.Control group was treated with tracheal intubation anesthesia,while experimental group with local anesthesia.Biochemical indicators,immune indicators,recovery indicators,and postoperative complications were compared between two groups.Results WBC,NEUT,and CRP in each group increased on the 1st day after surgery compared with before surgery,that of experimental group was lower than control group(P<0.05).All indicators decreased on the 4th and 7th day after surgery compared with the 1st day after surgery,that of experimental group was lower than control group(P<0.05).Levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in each group increased on the 1st,4th,and 7th day after surgery compared with the 1st day before surgery,those of experimental group were higher than control group(P<0.05).CD8^(+)level in each group on the 1st,4th,and 7th day after surgery decreased compared with the 1st day before surgery,that of experimental group was lower than control group(P<0.05).Recovery indicators of experimental patients were better than control group(P<0.05).Conclusion Local anesthesia can reduce effect on living and immune indicators of the body and promote postoperative recovery of rib fracture patients with internal fixation surgery.
