摘要
目的分析2019—2021年我院54例大量输血(MT)患者临床用血情况。方法回顾性分析2019年1月—2021年12月于本院治疗的54例MT患者临床资料。通过医院输血管理系统及医生工作站电子病历查询MT患者信息,并收集患者临床用血情况。分析MT患者一般临床特征及临床用血情况,比较患者输血前后纤维蛋白原(FIB)、凝血酶时间(TT)、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、国际标准化比值(INR)、血红蛋白(Hb)、血小板计数(PLT)、K^(+)、Ca^(2+)。结果MT患者中男38例(70.37%),女16例(29.63%);2019年MT患者有18例(33.33%),2020年21例(38.89%),2021年15例(27.78%);10~40岁MT患者有15例(27.78%),41~60岁有13例(24.07%),>60岁有26例(48.15%);MT患者疾病类型中外伤性疾病占比最高有25例(46.29%);MT患者输血量1600~5600mL,平均(2569.72±300.59)mL,其中输注红细胞有2例(3.70%),输注红细胞-血浆有31例(57.41%),输注红细胞-血浆-血小板-冷沉淀有4例(7.41%),输注血浆有17例(31.48%);患者输血后的TT短于输血前,Hb高于输血前,PLT低于输血前,差异有统计学意义(P<0.05);患者输血前、输血后的FIB、APTT、PT、INR、K^(+)、Ca^(2+)比较,差异无统计学意义(P>0.05)。结论2019-2021年我院54例MT患者临床用血主要为多种成分血液输注,对患者的血常规、凝血功能及电解质水平进行监测,有利于指导临床输血治疗。
Objective To analyze clinical blood use of 54 patients with massive transfusion(MT)in our hospital from 2019 to 2021.Methods The paper reviewed and analyzed clinical data of 54 MT patients in our hospital from January 2019 to December 2021,searched for information of MT patients through blood transfusion management system of hospital and electronic medical records at doctor's workstation,and collected clinical blood usage information,analyzed their general clinical characteristics and blood usage,compared fibrinogen(FIB),thrombin time(TT),activated partial thromboplastin time(APTT),prothrombin time(PT),international standardized ratio(INR),hemoglobin(Hb),platelet count(PLT),K^(+),and Ca^(2+)before and after transfusion.Results Among MT patients,there were 38 males(70.37%)and 16 females(29.63%).There were 18 cases(33.33%)of MT patients in 2019,21 cases(38.89%)in 2020,and 15 cases(27.78%)in 2021.There were 15 cases(27.78%)of MT patients aged 10~40,13 cases(24.07%)aged 41~60,and 26 cases(48.15%)over 60.There were 25 cases(46.29%)MT patients with the highest proportion of traumatic diseases,transfusion volume of MT patients was 1600~5600mL(2569.72±300.59)mL,of which 2 cases(3.70%)were treated with red blood cell infusion,31 cases(57.41%)with red blood cell plasma infusion,4 cases(7.41%)with red blood cell plasma platelet cryoprecipitate infusion,and 17 cases(31.48%)with plasma infusion.TT after blood transfusion of patients was shorter than before,Hb was higher than before blood transfusion,and PLT was lower than before blood transfusion,with statistical significance(P<0.05).There was no statistically significant difference in FIB,APTT,PT,INR,K^(+),and Ca^(2+)levels between before and after blood transfusion(P>0.05).Conclusion 54 patients with MT in our hospital mainly was treated with blood transfusions of various components from 2019 to 2021.Monitoring blood routine,coagulation function,and electrolyte levels of patients is beneficial for guiding clinical blood transfusion treatment.
