摘要
目的:比较动力性团体治疗与药物治疗对大学生社交焦虑障碍(SAD)的疗效,评价动力性团体治疗在大学生SAD中的临床应用价值。方法:纳入2020年6月~2021年12月芜湖市某高校在校大学生SAD患者80例,随机分为3组,其中动力性团体治疗组27例,采用动力性团体心理治疗,每周1次,共10次;药物治疗组25例,采用抗焦虑药物治疗10周;对照组28例,研究过程中不给予干预。各组在干预前及干预后采用社交焦虑量表(LSAS)、汉密尔顿焦虑量表(HAMA)和社会支持评定量表(SSRS)对所有患者进行评定,比较3组间的疗效差异。结果:动力性团体治疗组和药物治疗组的女性患者LSAS评分较治疗前降低(P<0.05),SSRS评分较治疗前升高(P<0.05)。动力性团体治疗组女性患者治疗后LSAS评分和HAMA评分较药物治疗组和对照组均降低(P<0.05),SSRS评分较对照组升高(P<0.05)。结论:动力性团体治疗能有效缓解女性大学生SAD患者的社交焦虑水平,可作为治疗SAD的一个重要手段。
Objective:To compare the efficacy of psychodynamic group therapy and drug therapy on social anxiety disorder(SAD)of college students,and to evaluate the value of clinical application of psychodynamic group therapy in SAD in college students.Methods:Eighty patients with SAD were recruited from a university in Wuhu from June 2020 to December 2021,and randomized to 3 groups.Students in the psychodynamic group(n=27)underwent psychodynamic group therapy once a week for 10 sessions in total.Twenty-five patients were included in the medication group,and treated with anti-anxiety drugs for 10 weeks,yet the participants in the control group(n=28)were not given any intervention during the study.The groups were tested with questionnaires using the Social Anxiety Scale(LSAS),Hamilton Anxiety Scale(HAMA)and Social Support Rating Scale(SSRS)before and after the intervention for all participants,and the differences in efficacy were compared among the 3 groups.Results:The LSAS scores were significantly lower,yet SSRS scores were notably higher in the female patients in psychodynamic group and medication group compared to pre-treatment(both P<0.05).The scores on LSAS and HAMA after treatment were decreased in the females in psychodynamic group,which was significantly lower than both the medication group and the control group(P<0.05),whereas the scoring on SSRS was markedly higher in psychodynamic group than in control group(P<0.05).Conclusion:Psychodynamic group therapy can effectively alleviate the social anxiety degree of female college students with SAD,and may be an important tool to treat SAD.
