双相情感障碍患者的自杀态度和心境障碍对其自知力水平的影响

The effect of suicidal attitude and mood disorderon the level of insight in patients with bipolar disorder

目的 调查双相情感障碍患者的自知力水平现况,分析抑郁症状、自杀态度、心境障碍对自知力水平的影响,为临床管理提供借鉴。方法 采用自知力与治疗态度问卷(ITAQ)、抑郁症症状快速自评量表(QIDS-SR)、自杀态度问卷(QSA)、心境障碍问卷(MDQ)和一般资料调查问卷对82例双相情感障碍患者进行调查,通过单因素和多因素分析探讨自知力水平的影响因素。结果 82例双相情感障碍患者ITAQ总分为(11.27±3.20)分,其中自知力水平损伤者30例。单因素分析结果显示,QSA评分、MDQ评分与患者自知力水平有关(P<0.05);Logistic回归分析结果显示,自杀态度和心境障碍为双向情感障碍患者自知力水平的独立影响因素(P<0.05)。结论 部分双相情感障碍患者存在自知力水平损伤,早期评估QSA评分、MDQ评分对改善患者自知力水平具有一定指导作用。

Objective To understand the level of insight in patients with bipolar disorder,and the effects of depressive symptoms,suicidal attitude and mood disorders on the level of insight,so as to provide reference for the clinical management.Methods The study investigated 82 patients with bipolar disorder using the Insight and Treatment Attitude Questionnaire(ITAQ),the Rapid Self-rating Depression Scale(QIDS-SR),the Suicide Attitude Questionnaire(QSA),the Mood Disorder Questionnaire(MDQ),and the General Information Questionnaire.Univariate and multivariate logistic regression analyses were conducted to identify the factors that influence the level of insight.Results The study analysed the total ITAQ score of 82 patients diagnosed with bipolar disorder,which was found to be(11.27±3.20).Among these patients,30 cases showed impairment of self-knowledge level.Univariate analysis revealed a significant relationship between QSA score and MDQ score with patients'self-knowledge level(P<0.05).Logistic regression analysis indicated that suicidal attitude and mood disorder were independent factors affecting the self-knowledge level of bipolar disorder patients(P<0.05).Conclusions Patients with bipolar disorder may experience a reduced level of self-awareness.Early assessment of QSA and MDQ scores can guide efforts to improve self-awareness.
