基于赋能理论的护理在老年脑卒中合并衰弱患者中的应用效果

Application effect of nursing based on empowerment theory in elderly stroke patients with frailty

目的 探讨基于赋能理论的护理在老年脑卒中合并衰弱患者中的应用效果。方法 选取2021年6月至2022年6月青岛大学附属医院收治的124例老年脑卒中合并衰弱患者为研究对象,将其按随机数字表法进行分组,各62例。对照组患者给予常规护理方案干预,观察组患者在常规护理的基础上给予基于赋能理论的护理方案干预。比较2组的心境状态、神经功能缺损情况、自理能力、衰弱管理依从性及自我效能。结果 干预4周和12周后,2组的简明心境状态量表评分、美国国立卫生院卒中量表评分、Barthel指数评分和自我效能感量表评分均优于干预前(P均<0.05),且观察组均优于同期对照组(P均<0.05);干预12周后,2组的上述评分均优于干预4周后(P均<0.05)。干预4周和12周后,观察组的衰弱管理依从良好率均高于同期对照组(P均<0.05)。结论 基于赋能理论的护理能够改善老年脑卒中合并衰弱患者的心境状态和神经功能缺损情况,提高其自理能力、衰弱管理依从性及自我效能。

Objective To explore the application effect of nursing based on empowerment theory in elderly stroke patients with frailty.Methods A total of 124 elderly stroke patients with frailty admitted to the Affiliated Hospital of Qingdao University from June 2021 to June 2022 were selected as the research subjects,and they were randomly divided into two groups using a random number table method,with 62 patients in each group.Patients in the control group were given routine nursing intervention,and patients in the observation group were given nursing intervention based on empowerment theory on the basis of routine nursing.The mood state,neurological impairment,self-care ability,frailty management compliance and self-efficacy of the two groups were compared.Results After 4 and 12 weeks of intervention,the Brief Profile of Mood States scores,National Institutes of Health Stroke Scale scores,Barthel Index scores,and General Self-Efficacy Scale scores of both groups were better than those before intervention(all P<0.05),and the above scores of the observation group were better than those of the control group during the same period(all P<0.05).After 12 weeks of intervention,the above scores in both groups were better than those after 4 weeks of intervention(all P<0.05).After 4 and 12 weeks of intervention,the good rate of frailty management compliance in the observation group was higher than that in the control group during the same period(both P<0.05).Conclusion Nursing based on empowerment theory can improve the mood state and neurological impairment of elderly stroke patients with frailty,enhance their self-care ability,frailty management compliance,and self-efficacy.
