雷火灸联合中药熏蒸在气滞血瘀型腰椎间盘突出症患者中的应用效果

Application effect of thunder fire moxibustion combined with traditional Chinese medicine fumigation on patients with lumbar disc herniation of qi stagnation and blood stasis type

目的 探讨雷火灸与中药熏蒸的联合应用在气滞血瘀型腰椎间盘突出症(LDH)患者中的价值。方法 以扬州市中医院于2018年6月至2022年5月收治的104例气滞血瘀型LDH患者为研究对象,采用简单随机抽样法对其进行分组,即对照组和观察组,每组52例。对照组和观察组分别接受常规护理和雷火灸联合中药熏蒸。比较2组患者的疼痛程度、腰椎功能改善情况及血清因子水平。结果 干预1周和干预2周后,2组的疼痛数字评分法评分均较干预前有所降低(P均<0.05);与同期对照组比较,观察组均更低(P均<0.05);2组的日本骨科协会评估治疗分数评分均较干预前有所升高(P均<0.05),且观察组均较同期对照组更高(P均<0.05)。干预1周和干预2周后,2组的肿瘤坏死因子α、转化生长因子β1和重组人基质金属蛋白酶3水平均较干预前有所降低(P均<0.05),且观察组均较同期对照组更低(P均<0.05)。结论 雷火灸与中药熏蒸联合应用可减轻气滞血瘀型LDH患者的疼痛程度,改善其腰椎功能和血清因子水平。

Objective To investigate the value of thunder fire moxibustion combined with traditional Chinese medicine fumigation in patients with lumbar disc herniation(LDH)of qi stagnation and blood stasis type.Methods A total of 104 patients with LDH of qi stagnation and blood stasis type admitted to Yangzhou Hospital of TCM from June 2018 to May 2022 were selected as the study subjects.They were grouped into a control group and an observation group using a simple random sampling method,with 52 cases in each group.Patients in the control group received routine care,and patients in the observation group received thunder fire moxibustion combined with traditional Chinese medicine fumigation.The degree of pain,improvement of lumbar spine function and serum factor level of the patients in the two groups were compared.Results After 1 and 2 weeks of intervention,the Numerical Pain Rating Scale scores of both groups decreased compared to before intervention(all P<0.05),and were lower in the observation group compared to the control group during the same period(both P<0.05).The Japanese Orthopaedic Association Assessment Treatment Score scores of both groups increased compared to before intervention(all P<0.05),and were higher in the observation group compared to the control group during the same period(both P<0.05).After 1 and 2 weeks of intervention,the levels of tumor necrosis factorα,transforming growth factorβ1,and recombinant human matrix metalloproteinase 3 in both groups decreased compared to before intervention(all P<0.05),and were significantly lower in the observation group compared to the control group during the same period(all P<0.05).Conclusion The combined application of thunder fire moxibustion and traditional Chinese medicine fumigation can reduce pain and improve lumbar spine function and serum factor levels in patients with LDH of qi stagnation and blood stasis type.
