常规心电图与同步12导联动态心电图检查在冠心病患者中的应用价值对比

Comparison of the value of conventional electrocardiography and synchronized 12-lead ambulatory electrocardiography in patients with coronary artery disease

目的 比较在冠心病患者诊断中应用常规心电图与同步12导联动态心电图检查的临床价值。方法 纳入2021年1月至2023年6月在我院接受检查的89例疑似冠心病患者作为研究对象,均给予常规心电图与同步12导联动态心电图检查,以冠状动脉造影作为金标准,比较常规心电图与同步12导联动态心电图检查的诊断冠心病的效能和合并心肌缺血符合率,并以心脏生理检查为金标准,比较常规心电图与同步12导联动态心电图检查诊断冠心病患者合并心律失常符合率和分型准确率。结果 同步12导联动态心电图诊断冠心病的准确率、灵敏度和特异度均高于常规心电图(P<0.05)。同步12导联动态心电图检查的诊断冠心病合并心肌缺血符合率高于常规心电图(P<0.05)。同步12导联动态心电图检查诊断冠心病合并心律失常符合率高于常规心电图(P<0.05)。同步12导联动态心电图检查诊断冠心病合并早搏、房性心动过速、室性心动过速及室上性心动过速等心律失常准确率高于常规心电图。结论 同步12导联动态心电图诊断冠心病效能高于常规心电图,且同步12导联动态心电图在冠心病合并心肌缺血、合并心律失常及心律失常分型方面准确率更高。

Objective Comparison of the clinical value of applying conventional electrocardiography(ECG)and synchronous 12-lead holter ECG in the diagnosis of patients with coronary heart disease.Methods Eighty-eight patients with suspected coronary artery disease who were examined in our hospital from January 2021 to June 2023 were included as study subjects,and they were given conventional ECG and synchronous 12-lead holter ECG.Using coronary angiography as the gold standard,the efficacy of conventional ECG and synchronous 12-lead holter ECG in diagnosing coronary heart disease and the coincidence rate of myocardial ischemia were compared,and the coincidence rate and classification accuracy of conventional ECG and synchronous 12-lead holter ECG in diagnosing arrhythmia in patients with coronary heart disease were compared.Results The accuracy,sensitivity and specificity of synchronous 12-lead holter ECG for the diagnosis of coronary heart disease were higher than those of conventional ECG(P<0.05).The compliance rate of synchronous 12-lead holter ECG for the diagnosis of coronary heart disease combined with myocardial ischemia was higher than that of conventional ECG(P<0.05).synchronous 12-lead holter ECG has a higher compliance rate than conventional ECG for the diagnosis of coronary artery disease combined with arrhythmia(P<0.05).The accuracy of synchronous 12-lead holter ECG in diagnosing arrhythmias of coronary heart disease combined with premature beats,atrial tachycardia,ventricular tachycardia and supraventricular tachycardia was higher than that of conventional ECG(P<0.05),and there was no statistically significant difference in the accuracy of the two diagnostic methods in the diagnosis of arrhythmias of sinus arrest,sinus bradycardia,sinus tachycardia and sinus block(P>0.05).Conclusion Synchronous 12-lead holter ECG is more effective than conventional electrocardiogram in the diagnosis of coronary artery disease,and synchronous 12-lead holter ECG is more accurate in coronary artery disease combined with myocardial ischemia,arrhythmia,and arrhythmia typing.
