CCCK-18CTRP-3miR-340-5p预测重症高血压性脑出血患者预后的价值

Value of CCCK-18,CTRP-3 and miR-340-5p in predicting the prognosis of patients with severe hypertensive intracerebral hemorrhage

摘要 目的探讨CCCK-18、CTRP-3、miR-340-5p在重症高血压性脑出血患者中的表达及相关性。方法选取哈尔滨医科大学附属第一医院2019-10—2021-10收治的80例重症高血压性脑出血患者为观察组,另选取同期健康体检者80例为对照组,比较对照组与观察组血清CCCK-18、CTRP-3、miR-340-5p水平,依据mRS评分将观察组患者分为预后良好组(55例)和预后不良组(25例),比较不同预后患者一般资料及血清CCCK-18、CTRP-3、miR-340-5p水平,Logistic回归分析重症高血压性脑出血患者预后的影响因素,Pearson相关性分析CCCK-18、CTRP-3、miR-340-5p之间的相关性,ROC曲线分析血清CCCK-18、CTRP-3、miR-340-5p单独及三者联合预测高血压性脑出血预后的价值。结果观察组血清CCCK-18高于对照组,CTRP-3、miR-340-5p低于对照组(P<0.05)。预后良好组血清CCCK-18低于预后不良组,CTRP-3、miR-340-5p高于预后不良组(P<0.05)。Logistic回归分析显示,血清CCCK-18、CTRP-3、miR-340-5p均是重症高血压性脑出血患者预后的影响因素(P<0.05)。血清CCCK-18与血清CTRP-3、miR-340-5p呈负相关(r=-0.385,r=-0.406,P<0.05),三者联合检测预测患者预后的AUC值较高(P<0.05)。结论重症高血压性脑出血患者CCCK-18呈高表达,CTRP-3、miR-340-5p呈低表达,其对高血压脑出血病情判断和预后具有重要意义。 Objective To investigate the expressions of CCCK-18,CTRP-3 and miR-340-5p in patients with severe hypertensive intracerebral hemorrhage.Methods Eighty patients with severe hypertensive intracerebral hemorrhage treated in the First Affiliated Hospital of Harbin Medical University from October 2019 to October 2021 were selected as the observation group,and 80 healthy people who underwent physical examination at the same time were selected as the control group.According to the modified Rankin scale(mRS).The levels of serum CCK-18,CTRP-3 and miR-340-5p in the control group and the observation group were compared,patients in the observation group were divided into good prognosis group(55 cases)and poor prognosis group(25 cases)according to mRS scores.General data and serum levels of CCCK-18,CTRP-3 and miR-340-5p of patients with different prognosis were compared.Logistic regression was used to analyze the prognostic factors in patients with severe hypertensive cerebral hemorrhage.Pearson correlation was used to analyze the correlation among CCCK-18,CTRP-3,miR-340-5p.ROC curve was used to analyze the prognostic value of serum CCCK-18,CTRP-3 and miR-340-5p alone or combined.Results The serum level of CCCK-18 in the observation group was higher than that in the control group,while CTRP-3 and miR-340-5p were lower than those in the control group(P<0.05).Serum CCCK-18 in good prognosis group was lower than that in poor prognosis group,and CTRP-3 and miR-340-5p were higher than those in poor prognosis group(P<0.05).Logistic regression analysis showed that serum CCCK-18,CTRP-3 and miR-340-5p were prognostic factors for patients with severe hypertensive intracerebral hemorrhage(P<0.05).The results of Pearson correlation analysis showed that serum CCCK-18 was negatively correlated with serum CTRP-3 and mi-340-5p(r=-0.385,-0.406,P<0.05).The combined detection of the three factors can predict the prognosis of patients with higher AUC value(P<0.05).Conclusion CCCK-18 is highly expressed in patients with severe hypertensive intracerebral hemorrhage,while CTRP-3 and miR-340-5p are low expressed,which is of great significance in judging the condition and prognosis of hypertensive intracerebral hemorrhage.
