牙周组织再生术联合Cervitec凝胶对牙周炎的疗效研究

目的:分析牙周组织再生术联合Cervitec凝胶治疗牙周炎的临床效果。方法:湖州市第一人民医院口腔科180例牙周炎患者(2017年1月至2019年1月)以随机数字法对患者进行分组,分为观察组和对照组各90例;对照组采用牙周组织再生术,观察组加用Cervitec凝胶治疗;对比较两组治疗前后牙周参数和龈沟液中菌群变化。结果:观察组与对照组患者治疗后牙龈指数[(0.13±0.05)mm,(0.24±0.08)mm]、菌斑指数[(0.83±0.08)分,(1.24±0.25)分]、探诊深度[(3.52±0.18)mm,(4.23±0.29)mm]、临床附着丧失[(4.37±0.25)mm,(5.03±0.37)mm]、龈沟出血指数[(1.88±0.17)分,(2.24±0.25)分],观察组的指标均更低,差异均有统计学意义(均P<0.05);观察组患者治疗后的总菌群、G-及G+菌群水平比对照组低,且治疗后观察组戈登链球菌、缓症链球菌、变黑普氏菌及牙龈卟啉单胞菌水平[分别为(0.21±0.07)×10^(4)·mL^(-1),(0.17±0.04)×10^(4)·mL^(-1),(0.71±0.08)×10^(4)·mL^(-1),(0.31±0.09)×10^(4)·mL^(-1)]明显低于对照组[分别为(0.95±0.09)×10^(4)·mL^(-1),(0.89±0.06)×10^(4)·mL^(-1),(1.14±0.10)×10^(4)·mL^(-1),(0.86±0.12)×10^(4)·mL^(-1)],差异均具有统计学意义(均P<0.05)。结论:牙周组织再生术及Cervitec凝胶结合治疗牙周炎效果良好,治疗后患者的龈沟液内菌群水平以及牙周检查参数等指标显著改善,值得应用。

Objective:To analyze the curative effect of periodontal tissue regeneration combined with Cervitec gel on periodontitis.Methods:A total of 180 patients with periodontitis treated in our hospital from January 2017 to January 2019 were enrolled in this study and randomized the observation group and control group,with 90 in each.The patients in the control group were treated with periodontal tissue regeneration,and those in the observation group with Cervitec gel.Periodontal parameters and microflora in gingival crevicular fluid before and after treatment were compared between the 2 groups.Results:After treatment,both groups showed improvements in teeth,and the gingival index[(0.13±0.05)mm,(0.24±0.08)mm],plaque index[(0.83±0.08)points,(1.24±0.25)points],probing depth[(3.52±0.18)mm,(4.23±0.29)mm],clinical attachment loss[(4.37±0.25)mm,(5.03±0.37)mm]and crevicular bleeding index[(1.88±0.17)points,(2.24±0.25)points]were lower in the observation group(P<0.05).The levels of total flora,G-and G+flora in the observation group after treatment were lower than those in the control.The levels of Streptococcus Gordonii,Streptococcus blastoides,Prevotella nigricans and porphyromonas gingivalis in the observation group after treatment were[(0.21±0.07)×10^(4)·mL^(-1),(0.17±0.04)×10^(4)·mL^(-1),respectively.(0.71+0.08)×10^(4)·mL^(-1),(0.31-0.09)×10^(4)·mL^(-1) respectively.]significantly lower than those in the control[(0.95-0.09)×10^(4)·mL^(-1),(0.89-0.06)×10^(4)·mL^(-1),(1.14±0.10)×10^(4)·mL^(-1),(0.86±0.12)×10^(4)·mL^(-1),respectively],the difference was statistically significant(P<0.05).Conclusion:Periodontal tissue regeneration combined with Cervitec gel has a good effect in the treatment of periodontitis.After treatment,the level of the bacteria community in the gingival crevicular fluid and the periodontal examination parameters are significantly improved,which is worthy of application.
