基于引火归元的穴位贴敷联合针灸治疗卒中后阴虚火旺型不寐的效果观察

Effect of acupoint herbal patching based on Yinhuo Guiyuan combined with acupuncture and moxibustion on insomnia patients with Yin deficiency fire type after stroke

目的探讨基于引火归元的穴位贴敷疗法联合针灸对卒中后阴虚火旺型不寐患者的治疗效果。方法抽取2022年3月至2023年3月河南省人民医院收治的150例卒中后阴虚火旺型不寐患者,采用抛硬币法随机分为观察组与对照组,每组75例。对照组接受针灸治疗,观察组在对照组基础上接受基于引火归元的穴位贴敷疗法,两组均治疗2个疗程。比较两组临床疗效,治疗前后中医症候积分、睡眠质量[匹兹堡睡眠质量指数(PSQI)]变化情况,同时观察两组治疗期间不良反应发生情况。结果治疗2个疗程后,观察组总有效率(90.67%,68/75)优于对照组(78.67%,59/75),P<0.05;治疗2个疗程后,两组中医症候积分及PSQI评分均下降,且观察组下降更明显(P<0.05);两组治疗期间不良反应发生率比较差异未见统计学意义(P>0.05)。结论应用基于引火归元的穴位贴敷疗法联合针灸治疗卒中后阴虚火旺型不寐患者可提高其临床疗效,减轻临床症状并改善睡眠质量,且具有良好的安全性。

Objective To investigate the effect of acupoint herbal patching based on Yinhuo Guiyuan combined with acupuncture and moxibustion on insomnia patients with Yin deficiency fire type after stroke.Methods A total of 150 patients with post-stroke insomnia of Yin deficiency fire type admitted to Henan Provincial People’s Hospital from March 2022 to March 2023 were selected,and they were divided into an observation group and a control group using the coin toss method,with 75 cases in each group.The patients in the control group received acupuncture and moxibustion treatment,and the patients in the observation group received acupoint herbal patching based on Yinhuo Guiyuan combined with acupuncture and moxibustion treatment.Both groups were treated for two courses.The clinical effects of the two groups were compared.The preoperative and postoperative traditional Chinese medicine symptom score and sleep quality assessed by Pittsburgh sleep quality index(PSQI)were compared between the two groups.At the same time,the adverse reactions during the treatment of the two groups were observed.Results After two courses of treatment,the total effective rate of the observation group(90.67%,68/75)was higher than that of the control group(78.67%,59/75),P<0.05.After two courses of treatment,the traditional Chinese medicine symptom score and PSQI score of the two groups decreased,and the observation group showed a more significant decrease(P<0.05).There was no statistical significant difference in adverse reactions between the two groups during treatment(P>0.05).Conclusions The application of acupoint herbal patching based on Yinhuo Guiyuan combined with acupuncture and moxibustion in the treatment of post-stroke insomnia of Yin deficiency fire type can improve clinical efficacy,reduce clinical symptoms and improve sleep quality,with good safety.
