腕踝针联合改良肩胛骨控制训练对锁骨骨折术后患者疼痛程度及肩关节功能的影响

Effect of wrist-ankle acupuncture combined with modified scapula control training on pain degree and shoulder joint function in patients after clavicle fractures surgery

目的探讨腕踝针联合改良肩胛骨控制训练对锁骨骨折术后患者疼痛程度及肩关节功能的影响。方法抽取2021年9月至2022年11月郑州市骨科医院收治的72例锁骨骨折患者为研究对象,按照随机数字表法分为对照组和观察组,每组36例。对照组给予常规康复治疗,观察组在对照组基础上实施腕踝针联合改良肩胛骨控制训练,两组患者均维持8周治疗。对比两组患者的疼痛程度[视觉模拟评分法(VAS)评分]、肩关节活动度、肩关节功能(Constant-Murley评分)、日常生活能力[Barthel指数(BI)]。结果治疗8周后,观察组VAS评分[(2.15±0.73)分]低于对照组[(4.32±1.04)分],P<0.05。治疗8周后,观察组肩关节前屈[(163.54±8.25)°]、后伸[(39.18±2.45)°]、外展[(158.15±10.89)°]及Constant-Murley评分[(89.53±5.84)分]、BI[(60.21±8.31)分]均高于对照组[(147.59±6.77)°、(34.11±1.78)°、(146.25±7.78)°、(74.32±4.11)分、(47.77±7.24)分],差异有统计学意义(P<0.05)。结论采用腕踝针联合改良肩胛骨控制训练,可有效减轻锁骨骨折术后患者的疼痛程度,改善其肩关节活动度,进一步提高锁骨骨折术后患者肩关节功能及日常生活能力。

Objective To investigate the effect of wrist-ankle acupuncture combined with modified scapula control training on the pain degree and shoulder joint function in patients with clavicle fractures after surgery.Methods A total of 72 patients with clavicle fractures who were admitted to Zhengzhou Orthopedics Hospital from September 2021 to November 2022 were selected as the research objects,and they were divided into control group and observation group according to the random number table method,with 36 cases in each group.The control group were given conventional rehabilitation treatment,and the observation group were given wrist-ankle acupuncture combined with modified scapula control training based on the conventional rehabilitation.Both groups were intervened for 8 weeks.The pain degree assessed by visual analogue scale(VAS)score,shoulder joint range of motion,shoulder joint function assessed by Constant-Murley score,and daily living ability evaluated by Barthel index(BI)of the two groups were compared.Results After 8 weeks of treatment,the VAS score of the observation group(2.15±0.73)was lower than that of the control group(4.32±1.04),P<0.05.The shoulder joint flexion(163.54±8.25)°,extension(39.18±2.45)°,abduction(158.15±10.89)°in the observation group were higher than the(147.59±6.77)°,(34.11±1.78)°,(146.25±7.78)°in the control group,P<0.05.The Constant-Murley score(89.53±5.84)and BI(60.21±8.31)in the observation group were higher than those in the control group(74.32±4.11,47.77±7.24),and the difference was statistically significant(P<0.05).Conclusions Wrist-ankle acupuncture combined with modified scapula control training can effectively reduce the pain degree,promote the improvement of shoulder joint mobility,and further improve the shoulder joint function and activities of daily living of patients after surgery for clavicle fractures.
