摘要
目的探讨爱泼斯坦-巴尔病毒(EB病毒)感染患者淋巴细胞亚群表达水平和肝功能异常的关联性。方法抽取2018年1月至2020年12月安阳市妇幼保健院收治的EB病毒感染患者122例为研究组,以1∶1配比抽取同期健康体检者122例为对照组。检测所有纳入患者淋巴细胞亚群表达水平,评价研究组患者肝功能情况。比较对照组与研究组淋巴细胞亚群表达水平,比较研究组肝功能异常者与肝功能正常者淋巴细胞亚群表达水平。分析EB病毒感染患者淋巴细胞亚群表达水平与肝功能的相关性。结果研究组CD4^(+)水平低于对照组,CD3^(+)、CD8^(+)水平高于对照组(P<0.05)。研究组外周血淋巴细胞比例及计数高于对照组(P<0.05)。研究组中肝功能正常者57例,肝功能异常者65例;研究组肝功能异常者CD3^(+)、CD8^(+)水平高于肝功能正常者,CD4^(+)水平低于肝功能正常者(P<0.05)。研究组肝功能正常者淋巴细胞比例、淋巴细胞计数低于肝功能异常者(P<0.05)。Pearson分析结果显示,CD3^(+)T细胞(r=-0.429)、CD8^(+)T细胞(r=-0.328)和肝功能异常呈负相关,CD4^(+)T细胞(r=0.523)、淋巴细胞比例(r=0.376)、淋巴细胞计数(r=0.425)和肝功能异常呈正相关(P<0.05)。结论EB病毒感染患者引发的肝脏功能受损和T细胞免疫调节紊乱存在明显关联性,临床可结合EB病毒感染患者淋巴细胞的检测结果诊断肝功能异常状况。
Objective To investigate the correlation between the expression level of lymphocyte subsets and abnormal liver function in patients with Epstein-Barr(EB)virus infection.Methods A total of 122 patients with EB virus infection admitted to Anyang Maternal and Child Health Hospital from January 2018 to December 2020 were selected as the study group.A total of 122 healthy individuals who underwent physical examinations during the same period were selected as control group with the ratio of 1∶1.The expression levels of lymphocyte subsets of all selected patients were detected.The liver function of patients in the study group were assessed.The expression levels of lymphocyte subsets were compared between the control group and the study group.The expression levels of lymphocyte subsets were compared between patients with and without abnormal liver function in the study group.The correlation between the expression level of lymphocyte subsets and liver function in patients with EB virus infection was analyzed.Results The levels of cluster of differentiation 4^(+)(CD4^(+))in the study group was lower than that in the control group,while the levels of cluster of differentiation 3^(+)(CD3^(+))and cluster of differentiation 8^(+)(CD8^(+))were higher than those in the control group(P<0.05).The proportion and count of peripheral blood lymphocytes in the study group were higher than those in the control group(P<0.05).There were 57 patients with normal liver function and 65 patients with abnormal liver function in the study group.Compared with the patients with abnormal liver function,the patients with normal liver function had higher levels of CD3^(+)and CD8^(+),but lower level of CD4^(+)in the study group(P<0.05).The proportion and count of lymphocytes of patients with normal liver function were lower than those of patients with abnormal liver function(P<0.05).Results of Pearson analysis showed that CD3^(+)T cells(r=-0.429)and CD8^(+)T cells(r=-0.328)were negatively correlated with abnormal liver function,while CD4^(+)T cells(r=0.523),lymphocyte ratio(r=0.376)and lymphocyte count(r=0.425)were positively correlated with abnormal liver function(P<0.05).Conclusions There is a clear correlation between the impaired liver function and the disorder of T cell immune regulation in patients with EB virus infection.In clinical,the results of lymphocytes detection can be used in the diagnosis of abnormal liver function in patients with EB virus infection.
