彩色多普勒血流参数对孕晚期胎儿宫内生长受限的诊断价值

Value of color Doppler blood flow parameters in diagnosis of fetal growth restriction in late pregnancy

目的研究彩色多普勒血流参数对孕晚期胎儿宫内生长受限(FGR)的诊断价值。方法抽取2020年3月至2023年3月濮阳市妇幼保健院收治的78例孕晚期FGR孕妇作为FGR组,另抽取同期胎儿健康的78例孕晚期孕妇作为健康组。两组均进行彩色多普勒超声检查,比较两组胎儿生长发育参数、脐动脉彩色多普勒血流参数[搏动指数(PI)、阻力指数(RI)、收缩期末流速/舒张期末流速(S/D)]、大脑中动脉彩色多普勒血流参数、主动脉峡部彩色多普勒血流参数。结果 FGR组胎儿头围、双顶径、股骨径短于健康组(P<0.05)。FGR组胎儿脐动脉PI、RI、S/D水平高于健康组(P<0.05)。FGR组胎儿大脑中动脉PI、RI水平低于健康组(P<0.05)。FGR组胎儿主动脉峡部收缩末反流流速(ESRV)水平高于健康组,收缩期末流速/ESRV水平低于健康组(P<0.05)。结论孕晚期FGR胎儿生长发育参数及脐动脉、大脑中动脉、主动脉峡部彩色多普勒血流参数均出现变化,临床可根据彩色多普勒血流参数诊断孕晚期FGR。

Objective To investigate the value of color Doppler blood flow parameters in the diagnosis of fetal growth restriction(FGR)in late pregnancy.Methods A total of 78 pregnant women with FGR in late pregnancy who were admitted to Puyang Maternity and Child Care Centers from March 2020 to March 2023 were selected as the FGR group,and 78 pregnant women with healthy fetuses in late pregnancy during the same period were selected as the healthy group.The two groups underwent color Doppler ultrasound examination,the fetal growth and development parameters,the fetal color Doppler blood flow parameters of the umbilical artery,including pulsatility index(PI),resistance index(RI)and end-systolic flow velocity/end-diastolic flow velocity(S/D),and the fetal color Doppler blood flow parameters of the middle cerebral artery and the aortic isthmus were compared between the two groups.Results The fetal head circumference,biparietal diameter,and femoral diameter in the FGR group were shorter than those in the healthy group(P<0.05).The PI,RI,and S/D levels of the fetal umbilical artery in the FGR group were higher than those in the healthy group(P<0.05).The PI and RI levels of the fetal middle cerebral artery in the FGR group were lower than those in the healthy group(P<0.05).The level of fetal aortic isthmus end-systolic regurgitation velocity(ESRV)in the FGR group was higher than that in the healthy group,and the level of the ratio of end-systolic flow velocity to ESRV in the FGR group was lower than that in the healthy group(P<0.05).Conclusions Fetal growth and development parameters and color Doppler blood flow parameters of the umbilical artery,middle cerebral artery and aortic isthmus in women with FGR in late pregnancy show changes.FGR in late pregnancy can be clinically diagnosed based on color Doppler blood flow parameters.
