经腋窝入路单孔腔镜手术治疗甲状腺肿瘤的效果研究

Effect of transaxillary approach single aperture endoscopic surgery on thyroid tumors

目的分析经腋窝入路单孔腔镜手术治疗甲状腺肿瘤的临床效果。方法抽取2021年3月至2023年3月河南科技大学第一附属医院收治的甲状腺肿瘤患者76例,按随机数字表法分为对照组和研究组,每组38例。对照组采用经胸乳入路腔镜手术治疗,研究组采用经腋窝入路单孔腔镜手术治疗。比较两组围术期情况、炎症因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)]水平、美容效果[数字评分法(NSS)评分]及并发症发生率。结果研究组手术时间、住院时间短于对照组,术中出血量、引流量少于对照组(P<0.05)。术后3 d,研究组CRP、IL-6水平低于对照组(P<0.05);术后1、3、5 d,研究组NRS评分低于对照组(P<0.05);研究组术后NSS评分高于对照组(P<0.05)。两组并发症发生率比较差异未见统计学意义(P>0.05)。结论经腋窝入路单孔腔镜手术治疗甲状腺肿瘤患者,可缩短手术时间,减少引流量,减轻炎症反应,减轻疼痛程度、降低并发症发生率。

Objective To analyze the effect of transaxillary approach single aperture endoscopic surgery in the treatment of thyroid tumors.Methods A total of 76 patients with thyroid tumor treated in the First Affiliated Hospital of Henan University of Science and Technology from March 2021 to March 2023 were selected,and they were divided into control group and study group using a random number table method,with 38 cases in each group.The control group was treated by endoscopic surgery through chest and breast approach,while the study group was treated by transaxillary approach single aperture endoscopic surgery.The perioperative conditions,levels of inflammatory factors,including C-reactive protein(CRP)and interleukin-6(IL-6),degree of pain assessed by numerical rating scale(NRS)score,cosmetic effects assessed by numerical score systen(NSS),and incidence of complications were compared between the two groups.Results The operation time and hospital stay of the study group were shorter than those of the control group,and the amount of blood loss and drainage volume was less than that of the control group(P<0.05).Three days after surgery,the levels of CRP and IL-6 in the study group were lower than those in the control group(P<0.05).The NRS score of the study group was lower than that of the control group 1,3 and 5 days after operation(P<0.05).The NSS score of the study group was higher than that of the control group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions Transaxillary approach single aperture endoscopic surgery can shorten the operation time,reduce the drainage flow,alleviate inflammation,relieve pain,reduce the incidence of complications.
