美罗培南与亚胺培南西司他丁钠在重症肺炎治疗中的应用效果对比

Comparison of effect of meropenem and imipenem cilastatin sodium in the treatment of severe pneumonia

目的对比分析美罗培南与亚胺培南西司他丁钠在重症肺炎治疗中的应用效果。方法抽取2019年1月至2020年12月义马煤业集团股份有限公司总医院收治的重症肺炎患者78例,按照随机数字表法分为研究组与对照组,每组39例。对照组采用亚胺培南西司他丁钠治疗,研究组采用美罗培南治疗。对比两组疗效、住ICU时间、带呼吸机时间、血气指标[血氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))、氧合指数(OI)]、炎症因子[白细胞介素-6(IL-6)、C反应蛋白(CRP)]水平及不良反应发生率。结果研究组总有效率(92.31%,36/39)高于对照组(69.23%,27/39),P<0.05。研究组住ICU时间、带呼吸机时间短于对照组(P<0.05);治疗后,研究组PaCO_(2)水平低于对照组,PaO_(2)、OI高于对照组(P<0.05);治疗后,研究组IL-6、CRP水平低于对照组(P<0.05)。研究组不良反应发生率(7.69%,3/39)低于对照组(25.64%,10/39),P<0.05。结论与亚胺培南西司他丁钠比较,美罗培南在重症肺炎中疗效显著,可改善血气指标,缩短住ICU时间及呼吸机支持时间,降低炎症因子水平,还能降低不良反应发生率。

Objective To compare and analyze the efficacy of meropenem and imipenem cilastatin sodium in the treatment of severe pneumonia.Methods A total of 78 patients with severe pneumonia treated in General Hospital of Yima Coal Industry Group Co.,Ltd.from January 2019 to December 2020 were selected,and they were divided into study group and control group according to random number table method,with 39 cases in each group.The control group was treated with imipenem cilastatin sodium,while the study group was treated with meropenem.The therapeutic effect,intensive care unit(ICU)stay,ventilator time,blood gas indexes,including partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),oxygenation index(OI),levels of inflammatory factors,including interleukin-6(IL-6)and C-reactive protein(CRP),and incidence of adverse reactions of the two groups were compared.Results The total effective rate of the study group(92.31%,36/39)was higher than that of the control group(69.23%,27/39),P<0.05.Compared with the control group,the length of ICU stay and ventilator time in the study group were shorter(P<0.05).Compared with the control group,the PaCO_(2)level in the study group was lower,and the levels of PaO_(2)and PaO_(2)/FiO_(2)were higher after treatment(P<0.05).Compared with the control group,the levels of IL-6 and CRP in the study group were lower after treatment(P<0.05).The incidence of adverse reactions in the study group(7.69%,3/39)was lower than that in the control group(25.64%,10/39),P<0.05.Conclusions Compared with imipenem cilastatin sodium,meropenem is effective in the treatment of severe pneumonia.It can improve blood gas indexes,shorten ICU stay and ventilator time,reduce the levels of inflammatory factors,and reduce the incidence of adverse reactions.
