腰痛解凝汤配合穴位针灸对腰椎间盘突出症患者的临床疗效及自身免疫水平的影响

Effect of Yaotong Jiening Decoction combined with acupoint acupuncture and moxibustion on clinical efficacy and autoimmunity of patients with lumbar disc herniation

目的:观察腰痛解凝汤配合穴位针灸治疗腰椎间盘突出症(LDH)的疗效及对症状改善、自身免疫水平的影响。方法:选取2020年10月~2022年10月本院收治的110例LDH患者,遵随机数字表法分为55例对照组(常规西医治疗)和55例观察组(对照组基础上^(+)腰痛解凝汤配合穴位针灸治疗)。比较两组间疗效,中医症状积分,腰痛程度,功能障碍程度,T淋巴细胞亚群和免疫因子水平差异,记录两组不良反应发生情况。结果:两组治疗有效率分别为98.18%和87.27%,差异具有统计学意义(P<0.05);治疗后,两组中医症状积分均降低(P<0.05),且观察组低于对照组(P<0.05);治疗后,两组JOA评分均升高(P<0.05),ODI评分均降低(P<0.05),且观察组JOA评分高于对照组(P<0.05),ODI评分低于对照组(P<0.05);治疗后,两组CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)均升高(P<0.05),且观察组高于对照组(P<0.05);治疗后,两组IL-6和TNF-ɑ均降低(P<0.05),且观察组低于对照组(P<0.05);两组均未出现明显不良反应。结论:腰痛解凝汤配合穴位针灸治疗LDH患者疗效确切,能有效改善临床症状,增强患者自身免疫力。

Objective To observe the efficacy of Yaotong Jiening decoction combined with acupoint acupuncture in the treatment of lumbar disc herniation(LDH)and its effects on symptom improvement and autoimmunity level.Methods 110 patients with LDH who were treated in the hospital from October 2020 to October 2022 were selected and divided into 55 cases in control group(conventional western medicine treatment)and 55 cases in observation group(Yaotong Jiening decoction combined with acupoint acupuncture on the basis of control group)according to the random number table ethod.The efficacy,TCM symptoms scores,lumbago degree,degree of lumbago dysfunction,T lymphocyte subsets and immune factors were compared between the two groups.The adverse reactions were recorded in the two groups.Results The effective rates of treatment in the two groups were 98.18%and 87.27%(P<0.05).After treatment,the TCM symptoms scores in both groups were decreased(P<0.05),and the scores in observation group were lower than those in control group(P<0.05).After treatment,the JOA score was increased(P<0.05)while the ODI score was decreased(P<0.05),and the JOA score in observation group was higher than that in control group(P<0.05)while the ODI score was lower than that in control group(P<0.05).After treatment,the CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were increased in both groups(P<0.05),and the indicators in observation group were higher compared with those in control group(P<0.05).After treatment,the levels of IL-6 and TNF-ɑin both groups were reduced(P<0.05),and the levels were lower in observation group compared to control group(P<0.05).There were no obvious adverse reactions in the two groups.Conclusion Yaotong Jiening decoction combined with acupoint acupuncture has an exact efficacy in the treatment of patients with LDH,and it can effectively improve the clinical symptoms and enhance immunity.
