无创呼吸机联合高流量氧疗对合并呼吸衰竭的急性加重期慢性阻塞性肺疾病患者血气指标及疗效的影响

Effect of Noninvasive Ventilator Combined with High Flow Oxygen Therapy on Blood Gas Indexes and Curative Effect in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease Complicated with Respiratory Failure

目的:分析合并呼吸衰竭的急性加重期慢性阻塞性肺疾病(简称慢阻肺)患者应用无创呼吸机联合高流量氧疗的临床效果.方法:选择2021年3月至2023年3月本院收治的110例合并呼吸衰竭的急性加重期慢阻肺患者为研究对象.随机将其分为研究组(n=55,接受无创呼吸机联合高流量氧疗)和对照组(n=55,接受无创呼吸机辅助通气治疗).比较两组患者临床疗效,治疗前、后血气指标水平,并记录治疗期间并发症发生情况.结果:与对照组比较,研究组临床治疗总有效率和血氧分压(PaO_(2))、>90%血氧饱和度(SpO_(2))、氧合指数(OI)等血气指标水平均更高,间歇治疗时间更长,机械通气治疗及重症监护室住院时间均更短,动脉血二氧化碳分压(PaCO_(2))水平和并发症发生率均更低,差异均有统计学意义(P<0.05).结论:无创呼吸机联合高流量氧疗的临床疗效显著,在改善合并呼吸衰竭的急性加重期慢阻肺患者血气指标方面效果突出,且并发症少,值得临床推广.

Objective:To analyze the clinical effect of non-invasive ventilator combined with high-flow oxygen therapy in chronic obstructive pulmonary disease(COPD)patients with acute exacerbation and respiratory failure.Methods:A total of 110 patients with acute exacerbation of COPD complicated with respiratory failure in our hospital from March 2021 to March 2023 were selected in this study.They were randomly divided into the study group(n=55,receiving non-invasive ventilator combined with high flow oxygen therapy)and the control group(n=55,receiving non-invasive ventilator assisted ventilation).The clinical efficacy,the level of blood gas indexes before and after treatment,and the occurrence of complications during treatment were compared between the two groups.Results:Compared with the control group,the study group had higher clinical treatment total effective rate and higher level of blood gas indicators such as arterial partial pressure of oxygen(PaO_(2)),>90%oxygen saturation(SpO_(2)),oxygenation index(OI),etc;The intermittent treatment time was longer,mechanical ventilation treatment and intensive care unit hospitalization time were shorter,while arterial partial pressure of carbon dioxide(PaCO_(2))levels and incidence of complication were lower;There were statistically significant differences between the two groups(P<0.05).Conclusion:Non-invasive ventilator combined with high-flow oxygen therapy has a significant clinical effect,and has a prominent effect on improving blood gas indexes in COPD patients with acute exacerbation complicated with respiratory failure,and has few complications,which is worthy of clinical promotion.
