短暂性脑缺血发作患者氯吡格雷抵抗发生的相关因素

Factors Related to Occurrence of Clopidogrel Resistance in Patients with Transient Ischemic Attack

目的:探讨短暂性脑缺血发作(TIA)患者氯吡格雷抵抗发生的相关因素.方法:选取2019年1月至2022年1月本院收治的32例发生氯吡格雷抵抗的TIA患者为研究对象,将其纳入发生组;另选取本院同期收治的32例未发生氯吡格雷抵抗的TIA患者纳入未发生组.比较分析导致TIA患者氯吡格雷抵抗发生的相关因素.结果:发生组患者合并2型糖尿病、合并急性冠状动脉综合征占比高于未发生组,血清25-羟维生素D[25-(OH)D]水平低于未发生组,差异均有统计学意义(P<0.05);组间其他资料比较,差异无统计学意义(P>0.05).经多因素非条件Logistic回归分析,结果显示合并2型糖尿病、合并急性冠状动脉综合征、血清25-(OH)D水平低是TIA患者发生氯吡格雷抵抗的危险因素(OR>1,P<0.05).结论:TIA患者氯吡格雷抵抗发生可能与患者合并2型糖尿病、合并急性冠状动脉综合征、血清25-(OH)D水平低等因素有关,临床应对此进行干预,以减少氯吡格雷抵抗的发生.

Objective:To explore related factors of clopidogrel resistance in patients with transient ischemic attack(TIA).Methods:A total of 32 TIA patients who developed clopidogrel resistance admitted to our hospital from January 2019 to January 2022 were selected in this study and included in occurrence group.In addition,32 TIA patients who did not develop clopidogrel resistance admited to our hospital during the same period were selected as the non occurring group.The clinical data of all patients were reviewed,and related factors leading to clopidogrel resistance in TIA patients were analyzed.Results:Compared with non-occurrence group,the proportion of patients with type 2 diabetes and acute coronary syndrome was higher,and the level of serum 25 hydroxyvitamin D[25-(OH)D]was lower in the occurrence group,the differences were statistically significant(P<0.05).There were no differences in other data between two groups(P>0.05).Multivariate unconditional logistic regression analysis showed that the risk factors of clopidogrel resistance in TIA patients were type 2 diabetes,acute coronary syndrome and low serum 25-(OH)D level(OR>1,P<0.05).Conclusion:The occurrence of clopidogrel resistance in TIA patients may be related to patients with type 2 diabetes,acute coronary syndrome,and low serum 25-(OH)D levels.Clinical intervention should be carried out to reduce the occurrence of clopidogrel resistance.
