蔗糖铁对维持性血液透析患者血红蛋白达标率的影响

摘要 目的:探讨蔗糖铁对维持性血液透析(MHD)患者血红蛋白(Hb)达标率的影响.方法:选取2021年11月至2022年12月本院收治的98例MHD患者为研究对象.依据不同治疗方案将其分为治疗组(n=49)和常规组(n=49).治疗组采用蔗糖铁+促红细胞生成素(EPO)治疗,常规组仅采用EPO治疗.比较两组患者治疗后临床总有效率、治疗前后红细胞压积(Hct)、Hb、转铁蛋白饱和度(TSAT)、血清铁、铁蛋白、铁调素、C反应蛋白(CRP)、白细胞介素(IL)-6、丙二醛、超氧化物歧化酶(SOD)水平及不良反应发生率.结果:治疗组临床总有效率为95.92%(47/49),高于常规组83.67%(41/49),差异有统计学意义(P<0.05);治疗后治疗组Hb、Hct、血清铁、TSAT、铁蛋白水平均高于常规组,铁调素水平低于常规组,差异有统计学意义(P<0.05);治疗后两组CRP、IL-6、SOD、丙二醛水平及不良反应发生率比较,差异无统计学意义(P>0.05).结论:蔗糖铁治疗MHD患者疗效确切,可改善Hb水平及铁代谢状态,且不增加氧化应激、炎症反应,安全性较高. Objective:To investigate effects of iron sucrose on the standard-reaching rates of Hemoglobin(Hb)in maintenance hemodialysis(MHD)patients.Methods:A total of 98 patients with MHD admitted to our hospital were selected as the study subjects.They were divided into a treatment group(n=49)and a control group(n=49)according to different treatment regimens.The treatment group was treated with iron sucrose and erythropoietin(EPO),while the control group was treated with EPO alone.The total clinical effective rate,as well as the levels of hematocrit(Hct),Hb,transferrin saturation(TSAT),serum iron,serum ferritin,serum hemodulin,C-reactive protein(CRP),interleukin(IL)-6,malondialdehyde,and superoxide dismutase(SOD)were compared before and after treatment between the two groups.The incidence of adverse reactions between the two groups were compared.Results:The clinical efficacy of the treatment group was 95.92%(47/49),which was higher than that of the control group(83.67%),there were statistically significant differences(P<0.05).After the treatment,the levels of serum Hb,Hct,serum iron,TSAT,serum ferritin in the treatment group were higher than those in the control group,while the level of serum hemodulin was lower than that in the control group,with significant differences(P<0.05).After the treatment,there was no significant difference in the levels of CRP,IL-6,SOD,malondialdehyde,and the incidence of adverse reactions between the two groups(P>0.05).Conclusions:The iron sucrose in treating MHD patients is effective,and it can improve serum Hb levels and iron metabolism status without increasing oxidative stress and inflammatory response,which is highly safe.
