青少年与成年人接受流产后关爱服务后采取长效可逆避孕措施的使用率及其影响因素分析

Usage Rate of the Long-Acting Reversible Contraception in Adolescents and Adults Receiving Post-Abortion Care Services and Its Influencing Factors

目的:探究青少年与成年人接受流产后关爱(PAC)服务后,采取长效可逆避孕(LARC)措施的使用率及其影响因素.方法:选取2019年8月至2020年8月于本院进行人工终止妊娠术的80例青少年与成年患者作为研究对象.分析青少年与成年患者接受PAC服务后LARC使用率及其影响因素.结果:青少年与成年患者接受PAC服务后LARC使用率为26.25%;年龄在20~22岁和23~24岁的成年患者LARC使用率均高于15~19岁的青少年与成年患者;文化水平在高中及专科、本科及以上的青少年与成年患者LARC使用率均高于初中及以下的青少年与成年患者;人工终止妊娠术次数<3次的青少年与成年患者LARC使用率高于人工终止妊娠术次数≥3次的青少年与成年患者;瘢痕子宫妊娠的青少年与成年患者LARC使用率高于非瘢痕子宫妊娠的青少年与成年患者;差异均有统计学意义(P<0.05).多因素非条件Logistic回归分析显示,年龄<20岁、文化水平初中及以下、人工终止妊娠术≥3次、非瘢痕子宫妊娠是青少年与成年患者接受PAC服务后LARC使用率较低的主要影响因素.结论:青少年与成年患者接受PAC服务后可增加LARC使用率,而低龄、低文化、人工终止妊娠术次数过多、非瘢痕子宫妊娠均会阻碍青少年与成年人LARC的落实,提示需重点关注此类青少年与成年人人群接受PAC服务的情况.

Objective:To investigate the usage rate of the long-acting reversible contraception(LARC)in adolescents and adults receiving post-abortion care(PAC)services,and its influencing factors.Methods:A total of 80 adolescents and adults who underwent induced abortion in our hospital from August 2019 to August 2020 were selected as the research subjects.The usage rate of the LARC after PAC services and the influencing factors were analyzed.Results:The usage rate of the LARC after PAC services in adolescents and adults was 26.25%.The usage rates of the LARC in adults aged 20-22 and 23-24 were higher than that in adolescents aged 15-19.The usage rates of the LARC in adolescents with education level of high school and junior college,bachelor degree or above were higher than that in adolescents and adults with education level of middle school or below.The usage rate of the LARC in adolescents and adults undergoing<3 times of induced abortion was higher than that in adolescents and adults undergoing≥3 times induced abortion.The usage rate of the LARC in adolescents and adults with scar pregnancy was significantly higher than that in adolescents and adults without scar pregnancy.There were statistically significant differences(P<0.05).Multivariate unconditional logistic regression analysis found that age under 20 years old,education level of middle school or below,3 times or more frequent induced abortion and non-scar pregnancy were major influencing factors of low usage rate of the LARC.Conclusion:PAC services can increase the usage rate of the LARC in adolescents and adults.Low age,low education level,excessive induced abortion and non-scar pregnancy affect the implementation of the LARC.It is necessary to strengthen PAC services for adolescents and adults with these factors.
