摘要
目的:了解泽普县15岁以下中学入学新生结核病感染现状,为学校结核病防控工作提供科学依据。方法:对2021年泽普县15岁以下中学入学新生进行结核菌素试验,对检测结果进行分析,并采用logistic回归模型进行影响因素分析。结果:结核菌素试验阳性率为5.47%(248/4531)。其中,一般阳性率为3.38%(153/4531),中度阳性率为1.50%(68/4531),强阳性率为0.60%(27/4531)。不同年级新生结核菌素试验阳性率和强阳性率比较,差异有统计学意义(χ^(2)值分别为31.74、20.26,P<0.05);不同民族新生结核菌素试验阳性率比较,差异有统计学意义(χ^(2)=37.83,P<0.05);农村新生和城镇新生结核菌素试验阳性率比较,差异有统计学意义(χ^(2)=17.42,P<0.05)。多因素logistic回归分析结果显示,高一年级(OR=3.43)、汉族(OR=2.46)和城镇(OR=1.69)是入学新生结核感染的主要因素。结论:开展结核菌素试验可以了解新生结核病感染现况,识别学生感染结核病的主要因素,从而有针对性地采取防控措施,防止结核病在校园内传播流行。
OBJECTIVE To understand the current situation of tuberculosis infection among the<15 year old middle school stu⁃dents in Zepu County,and to provide scientific basis for the prevention and control of tuberculosis in schools.METHODS The tuber⁃culin test was carried out on the new middle school students<15 years old in Zepu County in 2021,and the test results were ana⁃lyzed,and the influencing factors were analyzed by logistic regression model.RESULTS The positive rate of tuberculin skin test was 5.47%(248/4531),the rate of general positive was 3.38%(153/4531),the rate of moderate positive was 1.50%(68/4531),and the rate of strong positive was 0.60%(27/4531).There were significant differences in the positive rate and strong positive rate among freshmen of different grades(χ^(2)=31.74,20.26,P<0.05).There was significant difference in the positive rate of tuberculin skin test among freshmen of different nationalities(χ^(2)=37.83,P<0.05).There was significant difference in the positive rate of tuberculin skin test between urban and rural freshmen(χ^(2)=17.42,P<0.05).The results of multivariate logistic regression analysis showed that the main factors of TB infection in freshmen were senior one(OR=3.43),Han nationality(OR=2.46)and town(OR=1.69).CONCLU⁃SION Carrying out tuberculin test can understand the current situation of new tuberculosis infection and identify the main factors of tuberculosis infection in students,so as to take targeted prevention and control measures to prevent the spread of tuberculosis in school.
