甲状腺功能检测预测母婴结局的价值分析

Value Analysis of Thyroid Function Test in Predicting Maternal and Infant Outcomes

目的:探讨甲状腺功能检测预测母婴结局的价值。方法:选取2019年7月—2022年7月麻城市人民医院收治的300例孕妇作为研究对象,根据甲状腺功能检测结果分为甲状腺功能正常组和甲状腺功能异常组,各150例。记录两组孕妇妊娠结局和新生儿结局,采用受试者工作特征曲线分析甲状腺功能指标预测母婴结局的价值。结果:甲状腺功能正常组妊娠糖尿病、妊娠高血压综合征、胎膜早破、产后出血发生率均低于甲状腺功能异常组,差异有统计学意义(P<0.001)。FT3、FT4、促甲状腺激素(TSH)及联合检测对孕妇不良妊娠结局均有一定预测价值,且以联合检测预测价值最高。甲状腺功能正常组胎儿窘迫、早产儿、新生儿窒息、低体重儿发生率均低于甲状腺功能异常组,差异有统计学意义(P<0.001)。FT3、FT4、TSH及联合检测对新生儿不良事件均有一定预测价值,且以联合检测预测价值最高。结论:甲状腺功能检测有助于预测母婴结局,值得临床应用并予以推广。

Objective:To explore the value of thyroid function test in predicting maternal and infant outcome.Methods:A total of300 pregnant women admitted to Macheng People's Hospital from July 2019 to July 2022 were selected as the study objects.According to the thyroid function test results,they were divided into normal thyroid function group and abnormal thyroid function group,with 150 cases each.The pregnancy outcome and neonatal outcome of the two groups were recorded,and the value of thyroid function index in predicting maternal and infant outcome was analyzed by subject working characteristic curve.Results:The incidences of gestational diabetes mellitus,gestational hypertension syndrome,premature rupture of membranes and postpartum hemorrhage in the normal thyroid function group were lower than those in the abnormal thyroid function group,and the differences were statistically significant(P<0.001).FT3,FT4,thyroid stimulating hormone(TSH) and combined test have certain predictive value for adverse pregnancy outcomes,and the combined test has the highest predictive value.The incidence of fetal distress,premature infants,neonatal asphyxia and low birth weight infants in the normal thyroid function group was lower than that in the abnormal thyroid function group,with statistical significance(P<0.001).FT3,FT4,TSH and combined test have certain predictive value for neonatal adverse events,and the combined test has the highest predictive value.Conclusion:Thyroid function test is helpful to predict maternal and infant outcome,and is worthy of clinical application and promotion.
