CRP/PAB、NLR辅助诊断良恶性胃溃疡的价值及恶性胃溃疡患者CRP/PAB水平的影响因素分析

Value of CRP/PAB and NLR in the Diagnosis of Benign and Malignant Gastric Ulcers and the Influencing Factors of CRP/PAB in Patients with Malignant Gastric Ulcers

目的:分析C反应蛋白与前白蛋白比值(CRP/PAB)、中性粒细胞与淋巴细胞比值(NLR)辅助诊断良恶性胃溃疡的价值及恶性胃溃疡患者CRP/PAB水平的影响因素。方法:选取2019年10月—2022年10月于孝感市中心医院行电子胃镜检查的242例患者作为研究对象,根据检查结果分为良性胃溃疡组、恶性胃溃疡组、健康对照组。比较三组CRP/PAB、NLR水平,绘制受试者工作特征曲线(ROC),分析CRP/PAB、NLR辅助诊断胃良恶性溃疡的价值。根据ROC曲线截断值将恶性胃溃疡患者分为低CRP/PAB组和高CRP/PAB组,分析恶性胃溃疡患者CRP/PAB水平的影响因素。结果:恶性胃溃疡组CRP/PAB、NLR水平高于健康对照组与良性胃溃疡组,差异有统计学意义(P<0.05)。CRP/PAB辅助诊断恶性胃溃疡有一定的准确性,NLR辅助诊断恶性胃溃疡的准确性较低(P<0.001)。高CRP/PAB组年龄≥65岁、进展期、有淋巴结转移占比高于低CRP/PAB组,差异有统计学意义(P<0.05)。结论:CRP/PAB对恶性胃溃疡的辅助诊断价值优于NLR,年龄、病理分期、淋巴结转移情况是恶性胃溃疡患者CRP/PAB水平的影响因素。

Objective:To analyze the value of C-reactive protein to prealbumin ratio(CRP/PAB) and neutrophil to lymphocyteratio(NLR) in the diagnosis of benign and malignant gastric ulcers and the influencing factors of CRP/PAB level in patients withmalignant gastric ulcers.Methods:A total of 242 patients who underwent electronic gastroscopy in Xiaogan Central Hospital fromOctober 2019 to October 2022 were selected as the study objects,and were divided into benign gastric ulcer group,malignantgastric ulcer group and healthy control group according to the examination results.The levels of CRP/PAB and NLR in the threegroups were compared,receiver operating characteristic curve(ROC) was drawn,and the value of CRP/PAB and NLR in thediagnosis of benign and malignant gastric ulcers was analyzed.Malignant gastric ulcer patients were divided into low CRP/PABgroup and high CRP/PAB group according to the cut-off value of ROC curve,and the influencing factors of CRP/PAB level inmalignant gastric ulcer patients were analyzed.Results:The levels of CRP/PAB and NLR in malignant gastric ulcer group werehigher than those in healthy control group and benign gastric ulcer group,and the difference was statistically significant(P<0.05).CRP/PAB had a certain accuracy in the diagnosis of malignant gastric ulcer,while NLR had a low accuracy(P<0.001).Theproportion of patients with ≥65 years of age,advanced stage and lymph node metastasis in high CRP/PAB group was higher thanthat in low CRP/PAB group,and the difference was statistically significant(P<0.05).Conclusion:CRP/PAB is superior to NLR inthe auxiliary diagnosis of malignant gastric ulcer.Age,pathological stage and lymph node metastasis are the influencing factors ofCRP/PAB level in patients with malignant gastric ulcer.
