腹腔镜胆囊切除术瑞芬太尼复合丙泊酚的麻醉效果研究

A study on the anesthetic effect of remifentanil combined with propofol in laparoscopic cholecystectomy

目的:探讨瑞芬太尼复合丙泊酚在腹腔镜胆囊切除术中的效果。方法:将在医院就诊的78例行腹腔镜胆囊手术患者随机纳入对照组(39例,实施舒芬太尼复合丙泊酚麻醉)与研究组(39例,实施瑞芬太尼复合丙泊酚麻醉)。观察两组麻醉结局、血流动力学指标水平、术后应激情况、术后不良反应发生情况。结果:经与对照组比较,研究组患者意识恢复时间、拔管时间、恢复自主呼吸时间、止痛时间、眼时间更早(P<0.05),拔管时的心率、动脉舒张压与动脉收缩压更低(P<0.05),同时麻醉复苏后肾上腺素与皮质醇水平更低(P<0.05),且患者麻醉苏醒后躁动、恶心呕吐、头晕头痛等不良反应的总发生率更低(P<0.05)。结论:瑞芬太尼复合丙泊酚应用在腹腔镜胆囊切除术中,可以降低患者术后不良反应与生理应激风险,同时利于稳定患者血流动力学并促进患者及早苏醒。

Objective:To analyze the effect of remifentanil combined with propofol in laparoscopic cholecystectomy.Method:Patients who underwent laparoscopic cholecystectomy were sequentially included in the control group(n=39,receiving sufentanil combined with propofol anesthesia)and the study group(n=39,receiving remifentanil combined with propofol anesthesia).Observe the anesthesia outcomes of two groups;The level of hemodynamic indicators;Postoperative stress situation;Postoperative adverse reactions.Result:Compared with the control group,the study group had earlier recovery time of consciousness,extubation time,recovery of spontaneous breathing time,pain relief time,and eye opening time(P<0.05).The heart rate,arterial diastolic pressure,and arterial systolic pressure were lower after establishing pneumoperitoneum and extubation (P<0.05).At the same time,the levels of adrenaline and cortisol were lower after anesthesia recovery(P<0.05),and the patients were restless,nauseous,and vomiting after anesthesia recovery The total proportion of adverse reactions such as dizziness and headache was lower(P<0.05).Conclusion:The combination of remifentanil and propofol has the ability to reduce the risk of postoperative adverse reactions and physiological stress in patients undergoing laparoscopic cholecystectomy,while also stabilizing patient hemodynamics and promoting early recovery.
