达格列净联合缬沙坦治疗糖尿病肾病的效果

Effects of Dapagliflozin Combined with Valsartan on Diabetic Nephropathy and Renal Function

目的探讨达格列净联合缬沙坦治疗糖尿病肾病的效果。方法选取120例糖尿病肾病患者,随机分为对照组和观察组,每组60例。对照组予以达格列净治疗,观察组给予达格列净联合缬沙坦治疗。比较2组临床疗效、治疗前后血糖水平[空腹血糖(FPG)、餐后2 h血糖(2 h PG)和糖化血红蛋白(HbA1c)水平]、蛋白尿情况[尿微量蛋白(UmAlb)、24 h尿蛋白定量和尿白蛋白排泄率(UAER)]及肾功能指标[血清胱抑素C(Cys C)、尿素氮(BUN)和同型半胱氨酸(Hcy)水平]。结果观察组和对照组治疗总有效率分别为93.33%和83.33%,观察组总有效率高于对照组(P<0.05)。治疗前,2组血糖水平、蛋白尿情况及肾功能指标比较差异均无统计学意义(P>0.05)治疗后,观察组FPG、2 h PG、HbA1c、UmAlb、24 h尿蛋白定量、UAER及血清Cys C、BUN和Hcy水平均低于对照组(P<0.05或P<0.01)。结论达格列净联合缬沙坦应用于糖尿病肾病患者,可降低患者血糖水平,减少尿蛋白含量,改善肾功能,临床疗效佳。

Objective To investigate the effects of Dapagliflozin combined with Valsartan on diabetic nephropathy and renal function.Methods 120 patients with diabetic nephropathy were selected and randomly divided into the control group and the observation group,60 cases in each group.The control group was treated with Dapagliflozin,and the observation group was treated with Dapagliflozin combined with Valsartan.Clinical effects,blood glucose levels,proteinuria conditions,and renal function indexes were compared between the two groups.Results The total response rate of the observation group was 93.33%,and the total response rate of the control group was 83.33%,which was higher than that of the control group(P<0.05);before treatment,there were no statistically significant differences in blood glucose levels,proteinuria and renal function indexes between the 2 groups(P>0.05);after treatment,FPG,2 h PG,HbA1c,UmAlb,24 h urine protein quantification,UAER,and serum Cys C,BUN,and Hcy levels in the observation group were lower than those in the control group(P<0.01 or P<0.05).Conclusion Dapagliflozin combined with Valsartan might lower blood glucose level,reduce the content of urinary protein and improve renal function in diabetic nephropathy,with relatively effective clinical efficacy.
