基于“浊邪害清”理论探析慢性鼻窦炎的病机与辨证治疗

Pathogenesis,Syndrome Differentiation and Treatment of Chronic Rhinosinusitis Based on the Theory of"Turbidity Evil Obstructing Qing"

慢性鼻窦炎(Chronic rhinosinusitis, CRS)是发生于鼻窦黏膜的慢性炎性疾病,叶天士“浊邪害清”理论认为湿热浊邪蒙蔽于上而阻遏清阳,可致清窍为之壅塞。文章基于“浊邪害清”理论探讨CRS的基本病机和中医辨证治疗。CRS的基本病机为浊邪上蒙、阻遏清阳、清气不升、浊邪不降、壅塞鼻窍,临床治疗应注重培土生金,升清降浊,芳香化浊,清热行瘀,以达治病求本之效。

Chronic rhinosinusitis(CRS)is a chronic inflammatory disease occurring in the mucosa of the nasal sinuses.According to the theory of“Turbidity Evil Obstructing Qing”by Ye Tianshi,damp-heat pathogenic factors can obstruct the clear yang,leading to blockage of the nasal passages.This paper the basic pathogenesis and traditional Chinese medicine(TCM)treatment principles of CRS based on this theory.The basic pathogenesis of CRS involves the retention of pathogenic factors obstructing the clear yang,resulting in the stagnation of clear qi and blockage of the nasal passages.Clinical treatment should focus on promoting the spleen′s ability to produce nourishing substances,elevating the clear and dispersing the turbid,aromatizing and eliminating the turbid,clearing heat and promoting blood circulation in order to effectively treat the root cause of the disease.
