自体血回输技术在体外循环下成人瓣膜置换术中的临床应用

Clinical Application of Autologous Blood Transfusion in Adult Valve Replacement under Cardiopulmonary Bypass

目的:分析自体血回输技术在体外循环下成人瓣膜置换术中的临床应用效果。方法:调阅蚌埠医科大学第一附属医院心脏外科病例,选出在2020年3月—2023年1月进行瓣膜置换术的成年患者共104例。均采取择期体外循环下瓣膜置换术治疗,以输血方式不同分组,其中52例实施异体输血,另外52例采取自体血回输。对比两组相关指标。结果:观察组术后凝血功能较对照组高(P<0.05);观察组输血不良反应低于对照组(P<0.05);观察组术中、术后RBC、血浆用量及总用量均少于对照组,输血费用更低(P<0.05);观察组术后机械通气时间、ICU停留时间、术后住院时间均短于对照组,24 h引流量低于对照组(P<0.05)。结论:自体血回输技术能够有效维持体外循环下成人瓣膜置换术患者的凝血功能,耗时短,可避免大量输血,能够降低输血费用,促使患者术后尽早康复。

Objective:To analyze the clinical effect of autologous blood transfusion in adult valve replacement under cardiopulmonary bypass.Methods:A total of 104 adult patients who underwent valve replacement from March 2020 to January 2023 were selected by reviewing the cases of cardiac surgery in the First Affiliated Hospital of Bengbu Medical University.All of them were treated by elective extracorporeal circulation valve replacement,and were divided into different types of blood transfusion.52 of them received allogeneic blood transfusion and the other 52 received autologous blood transfusion.Relevant indicators in the two groups were compared.Results:The coagulation function of the observation group was higher than that of the control group(P<0.05).The adverse reactions of blood transfusion in the observation group were lower than those in the control group(P<0.05).The amount of RBC,plasma and total amount in the observation group were lower than those in the control group,with lower blood transfusion cost(P<0.05).Postoperative mechanical ventilation time,ICU stay time and postoperative hospitalization time in the observation group were shorter than those in the control group,and 24 h drainage volume was lower than that in the control group(P<0.05).Conclusion:Autologous blood transfusion can effectively maintain the coagulation function of adult valve replacement patients under cardiopulmonary bypass,shorten the time,avoid massive blood transfusion,reduce the cost of blood transfusion,and promote the early recovery of patients after surgery.
