宫颈癌患者术后院内感染发生情况及其危险因素分析

Analysis of incidence and risk factors of postoperative nosocomial infection in patients with cervical cancer

目的 探讨宫颈癌患者术后院内感染发生情况及分析其危险因素。方法 选取162例宫颈癌根治术患者,记录医院感染率,并分析相关危险因素。结果 纳入的162例宫颈癌根治术患者共发生院内感染21例,发生率为12.96%;感染部位包括泌尿道感染10例,切口感染6例,呼吸道感染3例,盆腔感染2例。多因素Logistic回归分析显示:合并糖尿病(OR=4.158,95%CI:1.231,14.044)、手术时间(OR=5.302,95%CI:1.446,19.443)、引流管留置时间(OR=4.080,95%CI:1.507,11.042)、导尿管留置时间(OR=5.585,95%CI:2.092,14.909)及放化疗(OR=4.879,95%CI:1.757,13.547)是影响患者发生院内感染的危险因素。结论 宫颈癌患者术后院内感染发生率较高,其中合并糖尿病、手术时间、引流管留置时间、导尿管留置时间及放化疗是其主要危险因素,应做好针对性防范措施。

Objective To investigate the incidence of postoperative nosocomial infection in patients with cervical cancer and analyze its risk factors.Methods The nosocomial infection rate was recorded in 162 patients undergoing radical resection of cervical cancer,and the related risk factors were analyzed.Results A total of 21 cases of nosocomial infection occurred in 162 patients undergoing radical resection of cervical cancer,with an incidence of 12.96%.The infection sites included urinary tract infection(10 cases),incision infection(6 cases),respiratory infection(3 cases) and pelvic infection(2 cases).Multivariate Logistic regression analysis showed that patients with diabetes mellitus(OR=4.158,95%CI:1.231,14.044),operation time(OR=5.302,95%CI:1.446,19.443),drainage catheter retention time(OR=4.080,95%CI:1.507,11.042),catheter indwelling time(OR=5.585,95%CI:2.092,14.909) and chemoradiation(OR=4.879,95%CI:1.757,13.547) were the risk factors for nosocomial infection.Conclusion The incidence of postoperative nosocomial infection in patients with cervical cancer is high,and the main risk factors are diabetes,operation time,catheter retention time,catheter retention time and radiotherapy and chemotherapy.
