摘要
目的:探究93例胃癌患者中错配修复蛋白(MMR蛋白)的表达,并分析胃癌患者MMR蛋白表达缺失与其胃癌临床病理特征之间的联系以及其与Ki67和P53之间的相关性,以期为临床治疗提供理论依据。方法:采用免疫组织化学染色法测定93例胃癌组织4种MMR蛋白(MLH1、MSH2、MSH6和PMS2)的表达,分析临床病理特征和微卫星不稳定性(MSI)有无相关性,同时检测Ki67、P53蛋白,并分析与MSI的相关性。结果:在93例胃癌中dMMR组的缺失率达到27.96%,MLH1与PMS2均缺失20例。dMMR组胃癌中Ki67蛋白表达均≥30%,统计分析dMMR组与Ki67蛋白表达呈正相关(r=0.271,P<0.01),dMMR组与P53表达无相关性。结论:MLH1与PMS2在胃癌中有一定比例的成对缺失,dMMR组胃癌与年龄、性别、肿瘤大小、分化程度、肿瘤位置、TNM分期等均不相关,因MSI型胃癌中Ki67表达较高,进一步说明MSI与Ki67对评估胃癌的恶性程度有协同作用。
Objective:To study the expression of mismatch repair protein(MMR protein)in 93 patients with gastric cancer,and analyze the relationship between the loss of MMR protein expression and the clinicopathological features of gastric cancer,as well as its correlation with Ki67 and P53,in order to provide theoretical basis for clinical treatment.Methods:Immunohistochemical staining was used to determine the expression of four MMR proteins(MLH1,MSH2,MSH6 and PMS2)in 93 cases of gastric cancer,and to analyze whether the clinicopathological characteristics were correlated with microsatellite instability(MSI),while Ki67 and P53 proteins were detected and their correlation with MSI was analyzed.Results:In 93 cases of gastric cancer,the deletion rate of dMMR group reached 27.96%,and MLH1 and PMS2 were deleted in 20 cases.The expression of Ki67 protein in all gastric cancers in dMMR group was≥30%.Statistically,there was a positive correlation between the expression of Ki67 protein in dMMR group and that of P53 in dMMR group(r=0.271,P<0.01),but no correlation between the expression of p53 in DMMR group.Conclusion:There was a certain proportion of pectin deletion between MLH1 and PMS2 in gastric cancer,and there was no statistical significance in dMMR group(age,sex,tumor size,differentiation degree,tumor location,TNM stage,etc.).The high expression of Ki67 in MSI type gastric cancer further indicates that MSI and Ki67 have a synergistic effect on evaluating the malignancy of gastric cancer.
