托法替布治疗SAPHO综合征1例

Effectiveness of Tofacitinib in SAPHO Syndrome:A Case Report

患者男,26岁,颈部、上背部疼痛1年,伴手足疱疹1个月余。以脓疱病、胸肋关节受累为主要表现,曾多次被误诊为单纯皮肤疾病,经对症治疗效果均不佳,后至风湿科最终诊断为SAPHO综合征。前期予糖皮质激素口服,起效虽快,但难以维持疗效,激素撤减时病情复发,遂配合托法替布口服,随访半年内,激素减停,未再复发。

A 26-year-old male patient with neck and upper back pain for 1 year with herpes on his hands and feet for more than 1 month.With impetigo and involvement of sternocostal joint as the main manifestations,it was misdiagnosed as skin disease for many times,and the symptomatic treatment was not effective.Finally,SAPHO syndrome was diagnosed in rheumatology department.In the early stage,glucocorticoid was taken orally,although the effect was fast,it was difficult to maintain the curative effect.The disease recurred when the glucocorticoid was withdrawn,and then the disease was taken orally with Tofacitib.Within half a year follow-up,the glucocorticoid was stopped and there was no recurrence.
