H型高血压患者血Hcy与血脂四项及炎症因子关系及临床意义探讨

Study on the Relationship and Clinical Significance of Hcy,Lipids and Inflammatory Factors in Patients with H-type Hypertension

目的探究H型高血压血同型半胱氨酸(Hcy)与血脂四项及炎症因子关系。方法选择2020年3月~2022年3月本院诊治的126例H型高血压患者,均行血Hcy、血脂四项及炎症因子检测,按血Hcy水平不同分为50例Hcy≥20μmol/L的高Hcy组、36例Hcy在15~<20μmol/L范围的中Hcy组、40例Hcy在10~<15μmol/L范围的低Hcy组。比较三组各项指标差异,分析Hcy与血脂四项及炎症因子关系。结果低Hcy组低密度脂蛋白胆固醇(LDL-C)比高Hcy组低,高密度脂蛋白胆固醇(HDL-C)比高Hcy组高(P<0.05);三组总胆固醇(TC)、甘油三酯(TG)相比无差异(P>0.05)。低Hcy组白细胞介素10(IL-10)比中Hcy组及高Hcy组高,白细胞介素6(IL-6)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子α(TNF-α)比中Hcy组及高Hcy组低,且中Hcy组各指标优于高Hcy组(P<0.05)。Pearson相关性分析中,H型高血压患者血Hcy与LDL-C、IL-6、hs-CRP、TNF-α均呈正相关,与HDL-C、IL-10均呈负相关(P<0.05)。多元逐步回归分析显示,IL-10为抑制H型高血压患者血Hcy升高的保护因素,IL-6、hs-CRP、TNF-α为H型高血压患者血Hcy升高的高危因素(P<0.05)。结论H型高血压患者血Hcy与部分血脂指标及炎症因子之间关系密切,可为患者后续干预提供借鉴。

Objective to investigate the relationship between homocysteine homocysteine(Hcy),lipids and inflammatory factors in patients with type H hypertension.Methods one hundred and twenty-six patients with H-type hypertension from March 2020 to March 2022 were enrolled in this study,according to the different levels of HCY,50 patients with high HCY(Hcy≥20μmol/L),36 patients with moderate Hcy(Hcy 15-<20μmol/L)and 40 patients with low HCY(Hcy 10-<15μmol/L)were divided into three groups.The relationship between Hcy,blood lipids and inflammatory factors was analyzed.Results the Low-density lipoprotein of LDL-C in the low HCY group was lower than that in the high HCY Group,and the high-density lipoprotein of HDL-C in the low HCY group was higher than that in the high Hcy group(P<0.05)There was no difference in total cholesterol(TC)and triglyceride cholesterol(TG)among the three groups(P>0.05).The interleukin of Il-10 in the low HCY group was higher than that in the middle Hcy group and High Hcy Group.The levels of IL-6,hs-CRP and TNF-αin the low HCY group were lower than those in the middle Hcy group and high HCY group,and the levels of IL-6,hs-CRP and TNF-αin the high HCY group were lower than those in the middle HCY Group and interleukin Hcy Group,the indexes in Hcy Group were better than those in high HCY group(P<0.05).Pearson's correlation analysis showed that Hcy was positively correlated with LDL-C,IL-6,hs-CRP and TNF-α,and negatively correlated with HDL-C and Il-10(P<0.05).Multiple stepwise regression analysis showed that IL-10 was a protective factor to inhibit the increase of HCY in patients with H-type hypertension,while IL-6,hs-CRP and TNF-αwere high risk factors for the increase of HCY in patients with H-type hypertension(P<0.05).Conclusion there is a close relationship between serum Hcy and some indexes of blood lipids and inflammatory factors in patients with H-type hypertension.
