冠心病合并衰弱病人营养管理最佳证据总结

摘要 目的:检索、评价及汇总冠心病合并衰弱老年病人营养管理的最佳证据,为护理人员实施营养管理方案提供循证依据。方法:按“6S”模型系统,检索国内外专业数据库以及相关网站关于冠心病合并衰弱老年病人营养管理的证据,包括指南、共识、系统评价、证据总结,并按照临床指南研究与评价系统(AGREEⅡ)、澳大利亚Joanna Briggs Institute(JBI)循证卫生保健中心质量评价标准对指南、专家共识、系统评价进行质量评价,提取符合质量标准的文献中营养管理的证据,最后采用2014版澳大利亚JBI循证卫生保健中心证据分级与推荐级别系统,对提取的证据进行等级评定。检索时限为2012年1月1日—2022年9月15日。结果:共纳入21篇文献,其中指南3篇、专家共识4篇、系统评价14篇。从评估、饮食摄入、营养治疗、健康行为4个方面汇总了28条证据。结论:本研究总结的冠心病合并衰弱病人营养管理的最佳证据,可为临床医护工作者提供营养健康管理的参考,有利于预防和控制冠心病合并衰弱病人营养不良事件的发生,提高病人的生活质量。 Objective:To search,evaluate and summarize the best evidence for nutrition management of elderly patients with coronary heart disease complicated with frailty,so as to provide evidence-based evidence for nursing staff to implement nutrition management program.Methods:According to the"6S"model system,the evidence of nutritional management of elderly patients with coronary heart disease combined with frailty was retrieved from domestic and foreign professional databases and related websites,including guidelines,consensus,systematic reviews,summaries of evidence,the guidelines,expert consensus and systematic review were evaluated according to the Clinical Guidelines Research and Evaluation System(AGREEⅡ)and the Australian Joanna Briggs Institute(JBI)evidence-based Health Care Center quality evaluation criteria.Evidence on nutrition management meeting quality standards was extracted.Finally,the 2014 edition of the Australian JBI Evidence-based Health Care Center evidence grading and recommendation rating system was used to grade the extracted evidence.The search period is from January 1,2012 to September 15,2022.Results:A total of 21 literatures were included,including 3 guidelines,4 expert consensus and 14 systematic reviews.A total of 28 pieces of evidence were collected from four aspects:assessment,dietary intake,nutrition treatment and health behavior.Conclusions:The best evidence of nutrition management of patients with coronary heart disease combined with frailty summarized in this study can provide reference for clinical health care workers in nutrition and health management,which is conducive to preventing and controlling the occurrence of malnutrition events in patients with coronary heart disease combined with frailty,and improve the quality of life of patients.
