自体脂肪移植联合二氧化碳点阵激光对烧伤患者增生性瘢痕的修复效果观察 被引量:1

摘要 目的评价自体脂肪移植联合二氧化碳点阵激光对烧伤患者增生性瘢痕的修复效果。方法以郑州仁济医院整形外科2020年10月-2023年2月收治的54例烧伤后增生性瘢痕患者的临床资料展开回顾性分析,按照入院治疗时间先后进行分组,以2020年10月-2022年1月治疗的27例烧伤后增生性瘢痕患者为对照组,以2022年2月-2023年2月治疗的27例烧伤后增生性瘢痕患者为观察组。对照组患者均进行二氧化碳点阵激光治疗,观察组患者均进行自体脂肪移植联合二氧化碳点阵激光治疗。比较两组患者的治疗效果、瘢痕恢复情况、瘢痕血流灌注量以及不良反应发生率。结果治疗后,观察组患者的治疗有效率高于对照组(P<0.05);观察组温哥华瘢痕量表(VSS)评分低于对照组(P<0.05);观察组瘢痕血流灌注量低于对照组(P<0.05);治疗期间,观察组有8例患者发生不良反应,对照组6例,两组患者的不良反应总发生率相比无明显差异(P>0.05)。结论应用自体脂肪移植联合二氧化碳点阵激光治疗烧伤后增生性瘢痕可以提升治疗效果,有助于改善患者瘢痕色泽、瘢痕厚度、血管分布、柔软性以及瘢痕充血情况,使瘢痕消退,且具有安全性。 Objective To evaluate the repair effffect of autologous fat transplantation combined with carbon dioxide lattice laser on hypertrophic scars in burn patients.Method:A retrospective analysis was conducted on the clinical data of 54 patients with post burn hypertrophic scars admitted to the Plastic Surgery Department of Renji Hospital in Zhengzhou from October 2020 to February 2023.They were grouped according to the time of admission for treatment.27 patients with post burn hypertrophic scars treated from October 2020 to January 2022 were used as the control group,and 27 patients with post burn hypertrophic scars treated from February 2022 to February 2023 were used as the observation group.The control group patients were all treated with carbon dioxide lattice laser,while the observation group patients were treated with autologous fat transplantation combined with carbon dioxide lattice laser.Compare the treatment effifficacy,scar recovery,scar blood flflow perfusion,and incidence of adverse reactions between two groups of patients.After treatment,the effffective rate of treatment in the observation group was higher than that in the control group(P<0.05);The Vancouver Scar Scale(VSS)score of the observation group was lower than that of the control group(P<0.05);The scar blood flflow perfusion in the observation group was lower than that in the control group(P<0.05);During the treatment period,8 patients in the observation group experienced adverse reactions,while 6 patients in the control group.There was no signifificant difffference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:The application of autologous fat transplantation combined with carbon dioxide dot matrix laser in the treatment of hypertrophic scars after burns can improve the treatment effffect,help improve the scar color,scar thickness,vascular distribution,softness,and scar congestion in patients,make the scar disappear,and have safety.
