卵巢颗粒细胞瘤的影像表现

摘要 目的探讨卵巢颗粒细胞瘤(OGCT)的影像表现及临床特征,以提高对该肿瘤的认识及诊断水平。方法回顾性分析4例进行手术治疗且确诊为OGCT的CT、MRI影像学、临床及病理相关资料,并分析病变的影像学特征。结果 4例患者(61岁,73岁,68岁,79岁)共发现4个肿瘤,3例位于左侧卵巢,1例位于右侧卵巢,病变于CT、MRI均表现为囊实性肿块,2例实性部分为主,1例囊实性部分相当,1例囊性部分为主,实性部分于T1WI呈等或稍高信号,T2WI呈稍高信号,DWI病变呈高信号,囊性部分3例T1WI呈低信号、T2WI呈高信号,1例T1WI呈低-稍高不均匀信号、T2WI呈低-高混杂信号,2例T2WI呈“蜂窝状、海绵状”典型改变,2例病变T2*WI见低信号。增强扫描病变实性部分呈中度强化。结论OGCT有一定的影像学特征,若发现卵巢囊实性肿瘤,T2WI呈“蜂窝状、海绵状”典型改变,实性成分于T2WI呈等或稍高信号,并有出血征象时,结合患者临床病史,应考虑到该肿瘤的诊断。 Objective To investigate the imaging findings and clinical features of OGCT in order to improve the understanding and diagnosis of this tumor.MethodsCT,MRI,clinical and pathological data of 4 patients diagnosed with OGCT after surgical treatment were retrospectively analyzed,and the imaging features of the lesions were analyzed.ResultsWe totally find four tumors in four patients(61 ys,73 ys,68 ys,79 ys),three on the left ovary and the other on the right ovary.All the tumors have the appearance of solid cystic mass on CT and MRI,in which the proportion of solid components is different.The solid part of the tumor shows isointensity or slightly high intensity on T1WI and slightly high intensity on T2WI,high intensity on DWI,while the cystic part shows hypointensity on T1WI and hyper intensity on T2WI in 3 cases,hypointensity or slightly high intensity on T1WI and mixed hypointensity or hyperintensity on T2WI.Typical changes of"honeycomb and spongy"sign on T2WI and low signal on T2*WI were observed in 2 cases.After contrast,the solid part shows moderate enhancement.ConclusionThere are some specific imaging features of OGCT.According to clinical history,ovarian solid cystic tumors which show"honeycomb and spongy"sign and isointensity or slightly high intensity on T2WI should be considered to the diagnosis of OGCT.
